Pharmacology
Lamivudine and Zidovudine inhibit viral reverse transcriptase via DNA chain termination. Nevirapine directly binds to reverse transcriptase and blocks DNA polymerase activities.
Mechanism of action
Synergistically reduces viral load by inhibiting reverse transcriptase enzyme and preventing viral DNA replication.
Dosage
Adults: One tablet twice daily. Not recommended as initial therapy until initial 2-week lower dose phase is completed and tolerated.
Administration
Administer orally twice daily with water.
Missed dose
Take as soon as remembered unless it is almost time for the next dose.
Side effects
Pancreatitis, peripheral neuropathy, headache, nausea, bone marrow toxicity (severe anemia/neutropenia), skin rash, Stevens-Johnson syndrome, lactic acidosis, and hepatomegaly.
Precautions & warnings
Assess risk in chronic hepatitis B, hepatomegaly with steatosis, lactic acidosis, and severe renal or hepatic impairment.
Contraindications
Known hypersensitivity to any of the components. Contraindicated as initial therapy.
Drug interactions
Interactions may occur with acyclovir, ganciclovir, doxorubicin, fluconazole, methadone, ribavirin, probenecid, and valproic acid.
