Argatroban

Argatroban is a medicine known as an anticoagulant or blood thinner. It helps prevent and treat blood clots. It is used to reduce the risk of stroke and heart attack. It prevents and treats clot formation in the veins of your legs, lungs, brain and heart. Argatroban is commonly used in patients with irregular heart rhythm (atrial fibrillation) to prevent clot formation. It also reduces the risk of getting clots in people who have undergone knee or hip replacement surgeries. It is given as an infusion (drip) into a vein under the supervision of a doctor. You may need to take this medicine for many years, even for life in some cases. Do not stop taking it or change the dose without guidance from your doctor. It could quickly put you more at risk of having a heart attack, stroke or thrombosis. You can reduce your risk of having a blood clot by making changes to your lifestyle, such as not smoking, eating a healthy diet, getting regular exercise and losing weight if you need to. The most common side effect of Argatroban is shortness of breath and decrease in blood pressure. If you experience any symptoms, tell your doctor immediately. Other side effects include headache, diarrhea, cardiac arrest and chest pain. Argatroban needs to be treated with caution. Do not take it if you have severe kidney or liver problems, if you are currently bleeding or if you are taking other medicines to reduce blood clotting. You should not breastfeed while using this medicine. Unlike other anticoagulants, regular blood test (PT-INR) is not required while taking this medicine. Argatroban is a direct thrombin inhibitor. It works by reversibly binding to thrombin, a substance in the blood that causes blood clotting. This prevents the formation of harmful clots in the body.

Argatroban is a synthetic direct thrombin inhibitor which reversibly binds to the active thrombin site of free and clot-associated thrombin. It exerts its anticoagulant effects by inhibiting thrombin-catalysed or -induced reactions, e.g. fibrin formation; activation of coagulation factors V, VIII, and XIII; activation of protein C; and platelet aggregation.

Adult Dose: Intravenous Thrombocytopenia Indicated as an anticoagulant for prophylaxis or treatment of thrombosis in patients with heparin-induced thrombocytopenia Initial: 2 mcg/kg/min IV continuous infusion over 1-3 hours until steady-state aPTT is 1.5-3 times initial baseline value Not to exceed infusion rate of 10 mcg/kg/min Check aPTT and adjust dose until target aPTT is achieved Percutaneous Coronary Intervention Indicated as an anticoagulant in patients with or at risk for heparin-induced thrombocytopenia undergoing percutaneous coronary intervention (PCI) Initial: 25 mcg/kg/min IV infusion, AND A bolus of 350 mcg/kg IV over 3-5 minutes via large-bore IV line Check activated clotting time (ACT) 5-10 minutes after bolus dose is completed; procedure may proceed if ACT is >300 seconds If ACT is 450 seconds, decrease infusion rate to 15 mcg/kg/min, and check ACT 5-10 minutes later After therapeutic ACT (300-450 sec) achieved, continue this infusion dose for duration of procedure Hepatic impairment Approximately 4-fold decrease in clearance relative to those with normal hepatic function, so dosing modification warranted; monitor aPTT and adjust dosage as clinically indicated Thrombocytopenia: Decrease initial dose to 0.5 mcg/kg/min IV PCI: For patients with hepatic impairment undergoing PCI and who have HIT or are at risk for HIT, carefully titrate argatroban until desired level of anticoagulation achieved; use of drug in PCI patients with clinically significant hepatic disease or AST/ALT levels ?3 times the upper limit of normal should be avoided

[SAFE] Argatroban does not usually affect your ability to drive. It is a hospital only medicine. It is given by your nurse or doctor as an infusion (drip) into a vein. Notify your doctor if you see blood in your vomit, urine or stool (black, tarry stools or bright red blood). Inform your doctor if you are taking other blood thinners or medicines to prevent blood clots.

Active major bleeding. Severe hepatic impairment.

Increased risk of bleeding w/ antiplatelet agents, thrombolytics and other anticoagulants. Increased INR and prothrombin time w/ warfarin.

[UNSAFE] It is unsafe to consume alcohol with Argatroban.

[SAFE IF PRESCRIBED] Argatroban is generally considered safe to use during pregnancy. Animal studies have shown low or no adverse effects to the developing baby; however, there are limited human studies.

[CONSULT YOUR DOCTOR] Argatroban is probably unsafe to use during breastfeeding. Limited human data suggests that the drug may pass into the breastmilk and harm the baby.

[SAFE IF PRESCRIBED] Argatroban is safe to use in patients with kidney disease. No dose adjustment of Argatroban is recommended. [CAUTION] Argatroban should be used with caution in patients with liver disease. Dose adjustment of Argatroban may be needed. Please consult your doctor.
