Daptomycin

Daptomycin is an antibiotic that fights bacteria. It is used to treat severe infections of the skin, soft tissues, heart, and blood. It works by killing the bacteria that cause these problems. It is commonly used to treat critically ill patients admitted to the hospital. Daptomycin is given under the supervision of a healthcare professional. The dose depends on what type of infection you have, where it is in the body and how serious it is. You should have your injections at the same time each day to get the most benefit and you should keep on taking this medicine for as long as you are prescribed it, even if your symptoms quickly improve. If you stop taking it too early the infection may return or worsen. Some people may develop side effects like diarrhea, headache, dizziness, rash and decrease in blood pressure. These side effects are usually temporary and go away during treatment as your body adjusts to the medicine. Consult your doctor if these side effects bother you or does not go away. Before starting treatment with this medicine, you should tell doctor if you are allergic to any antibiotic or have any liver or kidney problems. Pregnant or breastfeeding mothers should consult their doctor before using it. Your doctor may get regular blood tests done to monitor kidney and liver function. Daptomycin is an antibiotic. It kills bacteria by destroying the bacterial cell membrane.

• Severe bacterial infections

Daptomycin is a cyclic lipopeptide antibacterial derived from the fermentation of Streptomyces roseosporus. It causes bacterial cell death by binding to bacterial membranes and disrupting the membrane potential; thus inhibiting the synthesis of protein, DNA and RNA. Daptomycin exhibits rapid concentration-dependent bactericidal effects against susceptible gm+ve bacteria in vitro.

Adult Dose: Intravenous Complicated skin and skin structure infections Adult: 4 mg/kg once daily for 7-14 days, by infusion over 30 min or by inj over 2 min. Staphylococcus aureus bacteraemia Adult: 6 mg/kg once daily for 2-6 wk, by infusion over 30 min. Child Dose: Staphylococcus Aureus Bacteremia Indicated for treatment of children aged 1-17 years with Staphylococcus aureus bacteremia 1 year 12-17 years: 7 mg/kg IV q24hr 7-11 years: 9 mg/kg IV q24hr 1-6 years: 12 mg/kg IV q24hr Duration of therapy: Up to 42 days Complicated Skin & Skin Structure Infections Indicated for the treatment of adult and pediatric patients (aged 1 to 17 yr) with complicated skin and skin structure infections (cSSSI) caused by susceptible isolates of Gram-positive bacteria 1 year 12-17 years: 5 mg/kg IV q24hr 7-11 years: 7 mg/kg IV q24hr 2-6 years: 9 mg/kg IV q24hr 1 to <2 years: 10 mg/kg IV q24hr Duration of therapy: Up to 14 days Renal Dose: Renal impairment Complicated skin & skin structure infections (cSSSI) CrCl >30 mL/min: 4 mg/kg IV q24hr CrCl 30 mL/min: 6 mg/kg IV q24hr CrCl <30 mL/min: 6 mg/kg IV q48hr Hemodialysis and CAPD: 6 mg/kg IV q48hr administer following hemodialysis on hemodialysis days

Common: • Rash • Decreased blood pressure • Headache • Dizziness • Breathlessness • Abnormal liver function tests • Urinary tract infection • Diarrhea • Increased creatine phosphokinase (CPK) level in blood

[SAFE] Daptomycin does not usually affect your ability to drive. Daptomycin is usually only given in the hospital for treating serious bacterial infections of the blood, heart, skin and soft tissues. It is given by a drip into a vein (intravenous infusion). Your doctor may monitor the health of your muscles before you start treatment and frequently thereafter. Diarrhea may occur as a side effect but should stop when your course is complete. Inform your doctor if it doesn't stop or if you find blood in your stools. Discontinue Daptomycin and inform your doctor immediately if you get any unusual tingling, burning or numbness of the hands or feet, tenderness, weakness or cramps. Inform your doctor if you are pregnant, planning to conceive, or breastfeeding.

Hypersensitivity

Enhanced adverse/toxic effects w/ HMG-CoA reductase inhibitors (e.g. simvastatin). NSAIDs (e.g. diclofenac, ibuprofen) may reduce excretion of daptomycin and increase risk of renal impairment. Synergistic antibacterial effects against staphylococci and enterococci w/ aminoglycosides (e.g. gentamicin, tobramycin), rifampicin or β-lactam anti-infectives (e.g. penicillins, cephalosporins).

[SAFE] Consuming alcohol with Daptomycin does not cause any harmful side effects.

[SAFE IF PRESCRIBED] Daptomycin is generally considered safe to use during pregnancy. Animal studies have shown low or no adverse effects to the developing baby; however, there are limited human studies.

[SAFE IF PRESCRIBED] Daptomycin is probably safe to use during breastfeeding. Limited human data suggests that the drug does not represent any significant risk to the baby.

[CAUTION] Daptomycin should be used with caution in patients with kidney disease. Dose adjustment of Daptomycin may be needed. Please consult your doctor. [SAFE IF PRESCRIBED] Daptomycin is probably safe to use in patients with liver disease. Limited data available suggests that dose adjustment of Daptomycin may not be needed in these patients. Please consult your doctor.
