Description
Dolutegravir belongs to a group of medicines called antiretrovirals. It is used to treat HIV (human immunodeficiency virus), the virus that can cause AIDS (acquired immunodeficiency syndrome). It prevents the multiplication of virus in human cells and clears up your infection. Dolutegravir is not a cure for HIV or AIDS and only helps to decrease the amount of HIV in your body. This helps to lower your risk of getting HIV-related complications and improves your lifespan. It is prescribed in combination with other HIV medicines. Your doctor will recommend the best medicines for you and will decide the doses that you need. Follow the instructions carefully, for all the medicines that you are given. It may be taken with or without food, preferably at a fixed time. Taking all these medicines regularly at the right times greatly increases their effectiveness and reduces the chances of HIV becoming resistant to them. It is important not to miss doses and to keep taking them until your doctor tells you it is safe to stop. Some common side effects include headache, insomnia (difficulty sleeping), dizziness, diarrhea, nausea, and fatigue. These are usually not serious but tell your doctor if they bother you or persist for a longer duration. Rarely, some people may experience a skin reaction or liver damage. Your doctor will closely monitor you for these in the initial period of treatment. Before taking it, tell your doctor if you have any skin problems or liver or kidney disease. While using it, you may need regular blood tests to check your blood counts and liver function. Avoid drinking alcohol as it may increase your risk of liver damage. Pregnant or breastfeeding mothers should consult their doctor before using this medicine. Do not have unprotected sex or share personal items like razors or toothbrushes, if you are HIV positive. Talk to your doctor about safe ways like condoms to prevent HIV transmission during sex. Dolutegravir is an anti-HIV medication. It prevents the multiplication of virus in human cells. This stops the virus from producing new viruses and clears up your infection.

Mechanism of action
Dolutegravir inhibits HIV integrase by binding to its active site and blocking the strand transfer step of retroviral DNA integration, thereby preventing viral replication.
Dosage
Adult Dose: Oral HIV-1 infection Adult: In combination with other antiretrovirals: In patient without resistance to integrase inhibitors: 50 mg once daily. In patient with resistance, or concomitant use with other medication (e.g. carbamazepine, rifampicin, tipranavir/ritonavir, efavirenz, nevirapine): 50 mg bid. Hepatic impairment Mild-to-moderate hepatic impairment (Child-Pugh A or B): No dosage adjustment required Severe hepatic impairment (Child-Pugh C): Not recommended Child Dose: Oral HIV-1 infection Child: In patient without resistance to integrase inhibitors: 6-40 kg: 50 mg once daily. 12-18 years ?40 kg: 50 mg once daily. Renal Dose: Renal impairment Plasma concentrations were decreased in subjects with severe renal impairment No dosage adjustment required for treatment-naïve or treatment-experienced and INSTI-naïve patients with mild, moderate, or severe renal impairment or for INSTI-experienced patients (with certain INSTI-associated resistance substitutions or clinically suspected INSTI resistance) with mild or moderate renal impairment Severe renal impairment in for INSTI-experienced patients with resistance: Not recommended; decrease in dolutegravir concentrations may result in loss of therapeutic effect and development of resistance
Administration
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Dolutegravir may be taken with or without food, but it is better to take it at a fixed time.

Precautions & warnings
[UNSAFE] Dolutegravir may cause side effects which could affect your ability to drive. Take Dolutegravir with or without food, preferably at the same time everyday. Do not skip any doses and finish the full course of treatment even if you feel better. Do not take antacids or calcium supplements an hour before or two hours after taking Dolutegravir. Your doctor may monitor your liver function regularly during treatment. Inform your doctor if you notice yellowing of eyes or skin, dark urine, or stomach pain. Inform your doctor if you are pregnant, planning pregnancy or breastfeeding.
Contraindications
Hypersensitivity. Lactation. Concomitant use with dofetilide.
Drug interactions
May increase plasma concentration of metformin. Decreased plasma concentration with nevirapine, oxcarbazepine, phenobarbital, primidone. Reduced absorption with antacids, laxatives, Fe, Ca or mineral supplements. Potentially Fatal: May increase plasma concentration and increase the risk of adverse effects (e.g. torsade de pointes) of dofetilide.
Food interactions
[CAUTION] Caution is advised when consuming alcohol with Dolutegravir. Please consult your doctor.
Use in pregnancy
[SAFE IF PRESCRIBED] Dolutegravir is generally considered safe to use during pregnancy. Animal studies have shown low or no adverse effects to the developing baby; however, there are limited human studies.
Use in lactation
[CONSULT YOUR DOCTOR] Dolutegravir is probably unsafe to use during breastfeeding. Limited human data suggests that the drug may pass into the breastmilk and harm the baby.
Renal / hepatic impairment
[SAFE IF PRESCRIBED] Dolutegravir is safe to use in patients with kidney disease. No dose adjustment of Dolutegravir is recommended. [CAUTION] Dolutegravir should be used with caution in patients with liver disease. Dose adjustment of Dolutegravir may be needed. Please consult your doctor.
