Description
Hydrochlorothiazide + Metoprolol is a combination medicine used for treating hypertension (high blood pressure). By lowering the blood pressure, it helps in preventing future heart attack and stroke. Early treatment with this medicine also improves chances of survival in patients of heart attack. Hydrochlorothiazide + Metoprolol is taken with or without food preferably in the morning to avoid frequent urination at night. Keep taking it for as long as advised by your doctor. Even if you feel well, do not stop this medicine on your own because high blood pressure often has no symptoms. If you stop taking it, your condition may get worse. Keeping active with regular exercise, reducing your weight and eating a healthy diet will also help control your blood pressure. Follow your doctor's advice while taking this medicine. Nausea, constipation, diarrhea, and fatigue are some common side effects of this medicine. Consult your doctor If any of these bother you, or get worse, or do not go away. Drink plenty of fluids while taking medicine to overcome muscle weakness, dry mouth, and extreme thirst. Dizziness or headache may occur due to low blood pressure, hence rise slowly from sitting position or avoid driving. Before taking it, let your doctor know if you have any liver or kidney problems. Pregnant or breastfeeding women should also consult their doctor for advice before taking this medicine. You also need to tell your doctor what other medicines you are taking especially those used to treat high blood pressure or heart conditions. You should have your blood pressure, kidney function, blood sugar level and the levels of salts such as potassium or magnesium checked regularly to make sure that this medicine is working properly.

Mechanism of action
Hydrochlorothiazide + Metoprolol is a combination of two blood-pressure lowering medicines: Metoprolol Succinate and Hydrochlorothiazide. Metoprolol Succinate is a beta blocker which works specifically on the heart to reduce heart rate and make the heart more efficient at pumping blood throughout the body. Hydrochlorothiazide is a diuretic which lowers blood pressure by removing extra water and certain electrolytes from the body. Over time it also relaxes blood vessels and improves blood flow.
Dosage
Adult Dose: Oral Hypertension metoprolol tartrate 50-100 mg and hydrochlorothiazide 25-50 mg PO daily in single or divided doses Hepatic impairment: Reduction in dose may be necessary. Severe impairment: avoid. Renal Dose: Renal impairment: CrCl (ml/min) Dosage Recommendation <30 Avoid
Administration
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Hydrochlorothiazide + Metoprolol may be taken with or without food, but it is better to take it at a fixed time.
Side effects

Precautions & warnings
[UNSAFE] Hydrochlorothiazide + Metoprolol may decrease alertness, affect your vision or make you feel sleepy and dizzy. Do not drive if these symptoms occur. It is a combination of two blood pressure lowering medications used for the treatment of high blood pressure. It can be taken with or without food. Do not stop taking Revelol 50 H Tablet ER suddenly as it can cause your blood pressure to rise suddenly thereby increasing the risk of heart attack and stroke. It may cause dizziness. If this happens to you, get up slowly when rising from a sitting or lying position. It can hide symptoms of low blood sugar if you're diabetic. Monitor your blood sugar levels regularly. Revelol 50 H Tablet ER may cause dehydration. Drink plenty of fluids and Inform your doctor if you develop extreme thirst, muscle weakness or very dry mouth.
Contraindications
2nd or 3rd degree AV block, sinus bradycardia, cardiogenic shock, congestive heart failure; anuria; systolic hypotension (<100 mmHg); severe kidney or liver failure; therapy resistant hypokalaemia and hyponatraemia; hypercalcaemia; symptomatic hyperuricaemia; lactation.
Drug interactions
Effects of tubocurarine may be prolonged while corticosteroids may increase the risk of hypokalaemia; may precipitate azotemia in renal patients; increased hypotensive effects with: alcohol, α-blockers, general anaesthetics, hydralazine, levodopa, MAOIs, methyldopa, nitrates, phenothiazines, ACE inhibitors, adrenergic neurone blockers, aldesleukin, alprostadil, antiotensin-II receptor antagonists, TCAs, anxiolytics and hypnotics, baclofen, calcium-channel blockers, clonidine, diazoxide, minoxidil, moxisylyte, moxonidine, sodium nitroprusside, tizanidine; hydrochlorothiazide increases plasma concentration of fluconazole; β-blockers may mask warning signs of hypoglycaemia with antidiabetics, increased hypoglycaemic effect of insulin; hypotensive effect antagonised by corticosteroids, NSAIDs, oestrogens, indomethacin, ketorolac; reduced metoprolol plasma concentration with barbiturates, rifampicin; increased metoprolol plasma concentration with cimetidine, citalopram, escitalopram, paroxetine, propafenone; hypokalaemia potentially caused by hydrochlorothiazide may antagonise action of lidocaine, mexiletine, reboxetine; increased risk of hypokalaemia with acetazolamide, amphotericin, corticosteroids, theophylline; increased risk of hyponatraemia with carbamazepine, chlorpropamide; increased risk of sensitivity with allopurinol (especially in renal impairment); increased risk of hypercalcaemia with calcium salts, toremifene, vitamin D; increased risk of hypermagnesaemia with ciclosporin (and risk of nephrotoxicity); absorption of thiazides reduced by colestipol and colestyramine (take at least 2 hr apart).
Food interactions
[UNSAFE] It is unsafe to consume alcohol with Hydrochlorothiazide + Metoprolol.
Use in pregnancy

Although there are limited studies in humans, animal studies have shown harmful effects on the developing baby. Use in lactation
[SAFE IF PRESCRIBED] Hydrochlorothiazide + Metoprolol is probably safe to use during breastfeeding. Limited human data suggests that the drug does not represent any significant risk to the baby.
Renal / hepatic impairment
[CAUTION] Hydrochlorothiazide + Metoprolol should be used with caution in patients with severe kidney disease. Dose adjustment of Hydrochlorothiazide + Metoprolol may be needed. Please consult your doctor. Use of Hydrochlorothiazide + Metoprolol is not recommended in patients with severe kidney disease. [CAUTION] Hydrochlorothiazide + Metoprolol should be used with caution in patients with liver disease. Dose adjustment of Hydrochlorothiazide + Metoprolol may be needed. Please consult your doctor.
