যক্ষ্মা প্রাপ্তবয়স্ক: PO/IM সক্রিয় যক্ষ্মা 5 মিগ্রা/কেজি/দিন। সর্বোচ্চ: 300 মিলিগ্রাম/দিন। সুপ্ত যক্ষ্মা 300 মিলিগ্রাম/দিন 6 মাস জন্য। ননটিউবারকুলাস মাইকোব্যাকটেরিয়াল ইনফেকশন 5 মিলিগ্রাম/কেজি/দিন অন্তত 12 মাস কালচার-নেগেটিভ স্পুটামের জন্য। সর্বোচ্চ: 300 মিলিগ্রাম/দিন। INH থেরাপির সময় তীব্র লিভারের রোগ বা হেপাটিক ক্ষতির ইতিহাস; অতি সংবেদনশীলতা আইসোনিয়াজিড সংবেদনশীল ব্যাকটেরিয়াতে মাইকোলোয়িক অ্যাসিডের সংশ্লেষণে বাধা দেয় যার ফলে অ্যাসিড-দ্রুততা নষ্ট হয় এবং ব্যাকটেরিয়ার কোষ প্রাচীরের ব্যাঘাত ঘটে। থেরাপিউটিক স্তরে, এটি সক্রিয়ভাবে ক্রমবর্ধমান অন্তঃকোষীয় এবং বহির্মুখী মাইকোব্যাকটেরিয়াম যক্ষ্মা জীবের বিরুদ্ধে ব্যাকটেরিয়াঘটিত। রেনাল বা হেপাটিক বৈকল্য; খিঁচুনি ব্যাধি; সাইকোসিসের ইতিহাস; নিউরোপ্যাথি বা পাইরিডক্সিনের ঘাটতির ঝুঁকিতে থাকা রোগী যেমন, ডায়াবেটিক, অ্যালকোহলিক, অপুষ্ট, ইউরেমিক, এইচআইভি সংক্রামিত। কালো এবং হিস্পানিক মহিলাদের জন্য হেপাটিক ফাংশনের যত্ন সহকারে পর্যবেক্ষণ করা প্রয়োজন। চিকিত্সার আগে এবং সময়কালে হেপাটিক ফাংশন পরীক্ষা করুন। গর্ভাবস্থা এবং স্তন্যদান। পেরিফেরাল নিউরাইটিস, অপটিক নিউরাইটিস; মনস্তাত্ত্বিক প্রতিক্রিয়া, খিঁচুনি, বমি বমি ভাব, বমি, ক্লান্তি, এপিগ্যাস্ট্রিক যন্ত্রণা, চাক্ষুষ ব্যাঘাত, জ্বর, ফুসকুড়ি, পাইরিডক্সিনের অভাব। সম্ভাব্য মারাত্মক: হেপাটোটক্সিসিটি।

Mechanism of action
Isoniazid inhibits the synthesis of mycoloic acids in susceptible bacteria which results in loss of acid-fastness and disruption of bacterial cell wall. At therapeutic levels, it is bacteriocidal against actively growing intracellular and extracellular Mycobacterium tuberculosis organisms.
Dosage
Adult Dose: Adult: PO/IM Active tuberculosis 5 mg/kg/day. Max: 300 mg/day. Latent tuberculosis 300 mg/day for 6 mth. Nontuberculous mycobacterial infections 5 mg/kg/day for at least 12 mth of culture-negative sputum. Max: 300 mg/day.
Side effects
Peripheral neuritis, optic neuritis; psychotic reactions, convulsions, nausea, vomiting, fatigue, epigastric distress, visual disturbances, fever, rash, pyridoxine deficiency. Potentially Fatal: Hepatotoxicity.
Precautions & warnings
Renal or hepatic impairment; convulsive disorders; history of psychosis; patients at risk of neuropathy or pyridoxine deficiency eg, diabetic, alcoholic, malnourished, uraemic, infected with HIV. Careful monitoring of hepatic function is necessary for black and hispanic women. Check hepatic function before and during treatment. Pregnancy and lactation.
Contraindications
Acute liver disease or history of hepatic damage during INH therapy; hypersensitivity.
