Description

Paracetamol (acetaminophen) is the most common over-the-counter painkiller and fever reducer in Bangladesh, sold under many brands including Napa, Ace and Renova.

Indications

Used to relieve fever, headache, toothache, body ache and cold-related pain. It is the most widely used painkiller and fever reducer in Bangladesh.

Pharmacology

Paracetamol relieves pain and fever by inhibiting prostaglandin synthesis in the central nervous system. It acts as an analgesic and antipyretic but has little anti-inflammatory effect.

Mechanism of action

Inhibits COX enzymes (mainly central COX-2/COX-3) to reduce prostaglandin production, dampening pain signalling and acting on the hypothalamic heat-regulating centre to lower fever.

Dosage

Adults: 500 mg–1 g every 4–6 hours. Maximum 4 g per day. Children: 10–15 mg/kg by body weight.

Administration

Oral. May be taken with or without food; take after food if it upsets the stomach. Swallow with enough water.

Side effects

Safe at normal doses. Overdose can cause severe liver damage. Consult a doctor for long-term use.

Precautions & warnings

Caution in severe liver or kidney disease, chronic alcohol use or malnutrition. Never exceed the maximum daily dose (4 g) and do not combine multiple paracetamol-containing products.

Contraindications

Use with caution in severe liver disease.

Drug interactions

Long-term use with warfarin may increase bleeding risk. Caution with liver-enzyme–inducing drugs. Regular alcohol use raises the risk of liver damage.

Food interactions

No significant food interaction, but regular alcohol increases the risk of liver damage — avoid it.

Use in pregnancy

Generally considered safe and often the analgesic of choice in pregnancy. Use the lowest effective dose on a doctor's advice.

Pregnancy Category: B — always consult a doctor before taking during pregnancy.

Use in lactation

Small amounts pass into breast milk but are considered safe for the infant; it can be used while breastfeeding.

Pediatric use

Dose children by weight at 10–15 mg/kg per dose every 4–6 hours. Syrup or paediatric drops are convenient.

Geriatric use

Safe at usual doses in the elderly; adjust the dose if liver or kidney function is reduced.

Renal / hepatic impairment

In severe liver or kidney impairment the dose may need to be reduced or the interval lengthened; consult a doctor.

Overdose effects

Overdose can cause nausea, abdominal pain and severe liver damage after 24–48 hours. Seek hospital care immediately if overdose is suspected; N-acetylcysteine is used as the antidote.

Storage conditions

Store below 30°C in a dry place, away from light. Keep out of the reach of children.
