Saquinavir

Saquinavir is an antiviral or antiretroviral medicine. It is used in the treatment of HIV infection. It stops the virus from producing new viruses and clears up your infection. Saquinavir should be taken with food, as this increases the absorption of the medicine into the body. You should take it in the dose and duration as advised by your doctor. Taking it at the same time every day will help you to remember to take it. Do not stop taking it until you have finished the complete course, even when you feel better. If you stop taking this medicine too early then infection may return or worsen. Some people may experience rash, headache, dizziness, and vomiting as side effects of this medicine. Please consult your doctor if these side effects persist for a longer duration. Inform your doctor if you have diabetes as it can increase sugar levels in the blood. Before using it, you should tell your doctor if you are allergic to any antibiotics or have any kidney or liver problems. You should also let your doctor know all other medicines you are taking as they may affect, or be affected by this medicine. Pregnant and breastfeeding mothers should consult their doctor before using it. Your doctor may monitor your liver function while taking this medication. Saquinavir is an anti-HIV medication. It works by interfering with an enzyme (protease), which is required by HIV-infected cells to make new viruses. This stops the production of new viruses.

Saquinavir is a selective, competitive, reversible HIV protease inhibitor that interferes w/ the formation of essential proteins and enzymes. It blocks virus maturation and causes non-functional, immature and non-infectious virions formation.

Adult Dose: Oral HIV infection Adult: In combination w/ other antiretrovirals: 1,000 mg bid w/ ritonavir 100 mg bid. Treatment-naive patients: 500 mg bid (w/ ritonavir 100 mg bid) for the 1st 7 days of treatment then increase to 1,000 mg bid. Child Dose: HIV Infection Indicated in combination with ritonavir and other antiretrovirals (ARTs) for treatment of HIV-1 infection 16 years: 1000 mg (plus ritonavir 100 mg) PO q12hr For patients already taking ritonavir 100 mg BID as part of their ART regimen, no additional ritonavir is needed

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Saquinavir is to be taken with food.

Common: • Rash • Headache • Dizziness • Vomiting • Crystals in urine • Nausea • Abdominal pain • Indigestion • Paresthesia (tingling or pricking sensation) • Throat pain • Increased liver enzymes • Diarrhea • Peripheral neuropathy (tingling and numbness of feet and hand) • Blood in urine • Cough • Taste change • Hypercholesterolemia (high cholesterol) • Increased triglyceride level in blood • Protein in urine

[CONSULT YOUR DOCTOR] It is not known whether Saquinavir alters the ability to drive. Do not drive if you experience any symptoms that affect your ability to concentrate and react. Saquinavir should be taken with food. Do not skip any doses and finish the full course of treatment even if you feel better. It may cause dizziness or sleepiness. Do not drive or do anything that requires concentration until you know how it affects you. Your doctor may check your liver function regularly. Inform your doctor if you develop abdominal pain, loss of appetite darkened urine or yellowing of the eyes or skin (jaundice). Inform your doctor if you are pregnant, planning pregnancy or breastfeeding.

Hypersensitivity. Congenital or documented acquired QT prolongation, complete AV block (including patients at high risk) w/o implanted pacemakers, electrolyte disturbance (particularly uncorrected hypokalaemia), clinically relevant bradycardia or heart failure w/ reduced left-ventricular ejection fraction, history of symptomatic arrhythmia. Severe hepatic impairment. Lactation. Concomitant admin w/ drugs that prolong QT or PR interval, sedative/hypnotics (triazolam, oral midazolam), HMG-CoA reductase inhibitors (simvastatin, lovastatin), ergot alkaloids, rifampicin, quetiapine, alfuzosin.

May increase plasma concentration of fusidic acid. Increased risk of bleeding w/ anticoagulants (e.g. rivaroxaban). May alter plasma concentrations of warfarin. Potentially Fatal: Potentially prolonged or increased sedation and resp depression w/ sedative/hypnotics (oral midazolam, triazolam). Increased risk of myopathy including rhabdomyolysis w/ HMG-CoA reductase inhibitors (simvastatin, lovastatin). Increased potential for acute ergot toxicity w/ ergot alkaloids (e.g. ergotamine, dihydroergotamine, ergonovine, methylergonovine). Increased risk of severe hepatotoxicity w/ rifampicin. Risk of coma w/ quetiapine. Risk of hypotension w/ alfuzosin. Additive effect on QT or PR interval prolongation w/ antiarrhythmics (amiodarone, bepridil, dofetilide, flecainide, systemic lidocaine, propafenone, quinidine) neuroleptics (e.g. pimozide, clozapine, haloperidol), sildenafil (if used for pulmonary arterial HTN), antidepressant (trazodone), some anti-infectives (clarithromycin, erythromycin, halofantrine, pentamidine), some antihistamines (terfenadine, astemizole).

[UNSAFE] It is unsafe to consume alcohol with Saquinavir.

[SAFE IF PRESCRIBED] Saquinavir is generally considered safe to use during pregnancy. Animal studies have shown low or no adverse effects to the developing baby; however, there are limited human studies.

[SAFE IF PRESCRIBED] Saquinavir is probably safe to use during breastfeeding. Limited human data suggests that the drug does not represent any significant risk to the baby.

[SAFE IF PRESCRIBED] Saquinavir is probably safe to use in patients with kidney disease. Limited data available suggests that dose adjustment of Saquinavir may not be needed in these patients.
