Trimipramine

Trimipramine is a dibenzoxazepine antidepressant. It is used to treat depression. It also helps to enhance and brighten up the mood and is considered effective in treating depression that is unresponsive to other treatments. Trimipramine works by increasing the level of chemical messengers in the brain that stabilizes and enhances the mood. It is better to take it before bedtime because it can make you feel drowsy. It can be taken with or without food, but you should take it at a fixed time each day for better efficacy. The dose and duration will be decided by your doctor so that you get the right amount to control your symptoms. The most common side effects of this medicine include increased heart rate, blurred vision, dryness in the mouth, difficulty in urination and constipation. Initially, this medicine may also cause a sudden drop in blood pressure, especially when you change positions. It may even cause dizziness and sleepiness, so do not drive or do anything that requires mental focus until you know how this medicine affects you. To lower the chance of dizziness, rise slowly if you have been sitting or lying down. It may also lead to weight gain in some people. Most of the common side effects tend to be mild and temporary. Your doctor may be able to suggest ways of preventing or reducing side effects if they bother you or do not go away. Serious side effects associated with this medicine are rare. Before taking this medicine, it is important to tell your doctor if you are taking or have recently taken any other medicines for the same or any other diseases. Pregnant and breastfeeding women should take this medicine with proper consultation and caution. This medicine is not known to be addictive, but you can experience additional side effects (withdrawal symptoms) if you stop taking it suddenly. If you notice any sudden mood change or get suicidal or self-harm thoughts, you must consult the doctor without delay. Also, remember to take this medicine strictly as advised by the doctor as an overdose of this medication may lead to a serious health emergency. Consumption of alcohol should be avoided as alcohol interacts with this medicine to cause serious health issues. Trimipramine is a tricyclic antidepressant. • Depression

Trimipramine is a dibenzazepine TCA with antimuscarinic and sedative properties. It prevents the neuronal reuptake of noradrenaline in the CNS.

Adult Dose: Oral Depression Adult: Initially 50-75 mg daily, increased gradually as necessary to 150-300 mg daily. May be given in divided doses during the day or as a single dose at night. Elderly: Initial 50-75 mg daily, increased gradually if necessary. Max: 100 mg daily. Child Dose: Depression 12 years: 50 mg/day initially; titrate to 100 mg/day

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Trimipramine may be taken with or without food, but it is better to take it at a fixed time.

If you miss a dose of Trimipramine, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

Common: • Increased heart rate • Weight gain • Dryness in mouth • Constipation • Difficulty in urination • Orthostatic hypotension (sudden lowering of blood pressure on standing)

[UNSAFE] Trimipramine may decrease alertness, affect your vision or make you feel sleepy and dizzy. Do not drive if these symptoms occur. It may take 4 to 6 weeks for Trimipramine to start working. Keep taking it as prescribed. It may cause sleepiness. Take it at bedtime and do not drive or do anything requiring concentration until you know how it affects you. Avoid consuming alcohol when taking Trimipramine as it may cause excessive sleepiness. Monitor your weight during the treatment as this medicine can cause weight gain and increased appetite. It may cause sudden drop in your blood pressure. Rise slowly if you have been sitting or lying down. Talk to your doctor if you notice sudden mood changes or develop suicidal thoughts.

Recent MI, cardiac arrhythmias, heart block; mania; porphyria, severe liver disease. Neonates and children. Lactation.

Decreased antihypertensive effects of guanethidine, guanfacine, debrisoquine, betanidine and possibly clonidine. Increased CNS depression with CNS depressants such as alcohol, sedatives, hypnotics or barbiturates. Increased trimipramine levels with protease inhibitors, SSRIs, selegiline, tramadol, quinidine, diltiazem and verapamil. Decreased trimipramine levels with barbiturates. Increased risk of arrhythmias with drugs that prolong QT intervals. Increased risk of serotonin syndrome with linezolid. Increased antimuscarinic adverse effects with nefopam. Risk of neurotoxicity and serotonin syndrome with lithium. Potentially Fatal: Not to be given concurrently with, or within 2 wk of cessation of, therapy with MAOI and MAOI should be restarted at least 7-14 days after trimipramine has been stopped. Exaggerated response such as hypertension, cardiac arrhythmias with sympathomimetic drugs.
