Description
Terlipressin is used in the treatment of bleeding esophageal varices (bleeding from dilated veins in the food pipe leading to your stomach). It narrows the walls of blood vessels and decreases the blood flow into the affected veins. Terlipressin is given by a healthcare professional. You should not self-administer this medicine at home. Your doctor will decide the dose and how often you should take them. It is given only to patients with severe or life-threatening bleeding in the food pipe (esophagus). Using of this medicine can cause some common side effects such as headache, slow heart rate, high blood pressure, and low blood pressure. Let your doctor know if any of these side effects does not go away or get worse. Before using this medicine, inform your doctor if you have any problems with your heart, liver, or kidneys. You should also tell your doctor all the other medicines you are taking. This medicine is not recommended for children and adolescents. Terlipressin works on the walls of blood vessels, causing them narrow and decrease the blood flow into the affected veins. This helps to stop or slow the bleeding.

Mechanism of action
Terlipressin, an inactive prodrug, is converted via enzymatic cleavage to lysine-vasopressin. The active vasopressin selectively causes splanchnic and extrarenal vasoconstriction by stimulation of V1 receptors on vascular smooth muscle, thereby reducing splanchnic blood flow and portal pressure. It also increases systemic mean arterial pressure and decrease heart rate. It has minimal action on V2 receptors responsible for its antidiuretic effects.
Dosage
Adult Dose: Intravenous Acute oesophageal variceal haemorrhage Adult: As powder and solvent for solution for inj: Initially, 1-2 mg, then 1 mg 4-6 hourly. Max duration of treatment: 72 hours. As solution for inj: Initially, 2 mg 4 hourly, maintain until bleeding is controlled for 24 hours or up to max of 48 hours. Subsequent doses may be reduced to 1 mg 4 hourly in patients weighing <50 kg or if adverse reaction occur. Hepatorenal syndrome type 1 Adult: Initially, 1 mg 6 hourly via slow bolus inj, may be increased up to max 2 mg 4-6 hourly if serum creatinine has not been reduced by at least 25% after 3 days. Max duration of treatment: 2 weeks.
Administration
Your doctor or nurse will give you this medicine. Kindly do not self administer.
Missed dose
If you miss a dose of Terlipressin, skip it and continue with your normal schedule. Do not double the dose.

Precautions & warnings
[CONSULT YOUR DOCTOR] It is not known whether Terlipressin alters the ability to drive. Do not drive if you experience any symptoms that affect your ability to concentrate and react. Terlipressin is given as an injection into veins under the supervision of a doctor. It makes you feel dizzy or lightheaded. To lower the chance of feeling dizzy or passing out, rise slowly if you have been sitting or lying down. Tell your doctor if you are pregnant, planning to become pregnant or are breastfeeding.
Contraindications
Unstable angina, recent MI, septic shock with low cardiac output. Pregnancy.
Drug interactions
Increases the hypotensive effect of non-selective β-blockers. Concomitant use of drugs known to induce bradycardia (e.g. propofol, sufentanil) may lower heart rate and cardiac output. May trigger ventricular arrhythmias, including Torsade de pointes, when used with drugs that can prolong QT interval (e.g. class IA and III antiarrhythmics, erythromycin, certain antihistamines and TCAs), or with drugs that may cause hypokalaemia or hypomagnesaemia.
Food interactions

Use in pregnancy

My doctor ask me to take vit k injection. I am 35 week pregnant. Why this injection is necessary? Your blood must be showing some abnormal results regarding clotting. This injections are given to prevent excessive bleeding during delivery I have one child.is there is any injection for prevent the pregnancy. But the issue is that she has been bleeding and the first bleeding happened on 17-Dec-16 and clot was 6cc and next bleeding on 05-Jan-16 and clot was 10cc and she has been taking medicines and injection like sustain & Tranexa . but in the recent test on 11th Jan-16 , bleeding has been increased to 55cc and now doctor advised for vitamin k injection for 3 days please advise as this bleeding should not continue she is completely on bed rest for the last three months Sometimes a defective foetus or pregnancy result in spontanoeous bleeding and abortion.
