Resmetirom is a thyroid hormone receptor-beta (THR-β) partial agonist used in conjunction with diet and exercise for the treatment of noncirrhotic nonalcoholic steatohepatitis (NASH) with moderate to advanced liver fibrosis (F2-F3).

Pharmacology

Resmetirom is a selective partial agonist of THR-β that activates THR-β receptors in the liver, reducing intrahepatic triglycerides and improving liver fibrosis. It is more selective for THR-β compared to THR-α, reducing cardiovascular effects.

Dosage

Adult Dose (Oral):

  • <100 kg: 80 mg once daily
  • ≥100 kg: 100 mg once daily
  • With moderate CYP2C8 inhibitors (<100 kg): 60 mg/day; (≥100 kg): 80 mg/day
  • Can be taken with or without food

  • Abdominal pain
  • Dizziness
  • Pruritus
  • Constipation

Precautions & warnings

  • Drug interaction with statins: Dose adjustment needed for atorvastatin, pravastatin, rosuvastatin, simvastatin
