Rabies Vaccine (Human) BP is an inactivated virus vaccine used for active immunization against rabies. It can be used for pre- and post-exposure immunization and is often used with rabies immunoglobulin for post-exposure immunization.
Indications
- Pre-exposure immunization (high-risk professionals: veterinarians, laboratory personnel, forestry workers)
- Post-exposure immunization (bite or contact with rabid animal)
Dosage
Pre-exposure Immunization (IM):
- Primary: 1 ml IM on D0, D7, D21 or D28; booster at 1 year
- Boosters: Every 5 years or when titre <0.5 IU/ml
Post-exposure Immunization (Non-immunized):
- IM (1-1-1-1-1): 1 ml on D0, D3, D7, D14, D28
- IM (2-1-1): 2 ml (1 ml each) on D0; 1 ml on D7 and D21
- ID (Thai Red Cross 2-2-2-0-1-1): 2x0.1 ml on D0, D3, D7; 0.1 ml on D28; 0.1 ml on D90 (optional)
- ID (WHO Modified 2-2-2-0-2): 2x0.1 ml on D0, D3, D7, D28
Post-exposure (Previously Immunized):
- If <5 years: 2 injections on D0 and D3
- If >5 years or incomplete: 5 injections (D0, D3, D7, D14, D28) + immunoglobulin if needed
Side effects
Side Effects (>10%):
- Injection site pain, redness, swelling, itching (30-74%)
- Nausea/vomiting (5-40%)
- Abdominal pain, diarrhea (5-40%)
- Headache, fatigue (5-40%)
- Low-grade fever, chills, muscle ache, dizziness (5-40%)
Precautions & warnings
- Immune complex reaction 2-21 days after HDCV booster doses (arthralgia, arthritis, angioedema)
- Caution in bleeding disorders, anticoagulant treatment, severely immunocompromised patients
- Do not use in confirmed rabies diagnosis
- Pre-exposure: Contraindicated in severe fever, febrile infection, acute disease
- Post-exposure: No contraindication (rabies is lethal)
