It is available in a 20% concentration, which is hyperoncotic and suitable for patients who need limited fluid and sodium intake . In Bangladesh, it is available as an infusion under various brand names.
Description
Albumin Human 20% is used to treat blood volume loss. It works by replacing the body fluids lost due to excessive trauma, bleeding, surgery, or kidney dialysis. Thus, it is helpful in treating low blood albumin levels. Albumin Human 20% must be administered under the supervision of a healthcare professional. You should not take it if you have any known allergy from this injection. Before taking it, inform the doctor you are suffering from any severe heart disease. Your doctor may check your blood pressure and ask you to get regular blood tests done while you are using this injection. The most common side effects include flushing (sense of warmth in the face, ears, neck, and trunk), nausea and fever. Inform your doctor if you are pregnant or breastfeeding or suffering from kidney diseases as a dose adjustment may be required. This injection is safe to use in patients suffering from liver diseases. Albumin Human 20% contains human albumin, a natural protein in blood. It works by replacing the blood or body fluids lost due to excessive bleeding, surgery, or kidney dialysis.
Indications
• Hypovolemia: To restore and maintain blood volume when there is a reduction in circulating blood volume. • Hypoalbuminemia: To correct severe albumin deficiency caused by illness or active blood loss. • Paracentesis due to Cirrhotic Ascites: To maintain cardiovascular function after removal of a large volume of ascitic fluid. • Ovarian Hyperstimulation Syndrome (OHSS): As a plasma volume expander in severe OHSS. • Adult Respiratory Distress Syndrome (ARDS): To correct fluid volume overload in combination with diuretics. • Burns: To correct severe albumin deficiency after 24 hours following a burn injury. • Hemodialysis: In patients undergoing long-term dialysis or those with fluid overload. • Cardiopulmonary Bypass: As a priming fluid.

Pharmacology
Human albumin is not a glycoprotein and has the lowest molecular weight (66,241 Daltons) of all plasma proteins, resulting in lower viscosity than other plasma proteins . Approximately 40% of albumin is in circulation; the remainder is in extravascular spaces of tissues (muscle, skin, intestine). The major function of albumin is osmotic regulation . Albumin 20% is hyperoncotic and draws fluid from interstitial spaces into the vascular compartment .
Mechanism of action
Human albumin increases intravascular oncotic pressure and causes movement of fluids from interstitial into intravascular space. Human albumin solutions are available in various concentrations. Solutions containing 5% human albumin are usually used in hypovolemic patients, whereas more concentrated 25% solutions are recommended in patients in whom fluid and sodium intake must be minimised e.g. patients with hypoproteinaemia or cerebral oedema or in paediatric patients.
Dosage
Adult Dose: Adult: Initially, 25 g of albumin, adjusted according to patient's response. Usual rates of infusion: Up to 1-2 ml/minute (20% solution). Intravenous Hypoproteinaemia Adult: Up to 2 g/kg daily. Usual rates of infusion: Up to 1-2 ml/minute (20% solution). Special Populations: Volume admin and rate of infusion must always be individualised according to situation and response. Usual rates of infusion are up to 1-2 ml/min (20% soln). Child Dose: Child: Up to 1 g/kg, adjusted according to patient's response. Usual rates of infusion: Up to 1-2 ml/minute (20% solution). Intravenous Neonatal hyperbilirubinaemia Child: 1 g/kg of albumin before exchange transfusion. Usual rates of infusion: Up to 1-2 ml/minute (20% solution). Special Populations: Volume admin and rate of infusion must always be individualised according to situation and response. Usual rates of infusion are up to 1-2 ml/min (20% soln).

Missed dose
• This medicine is administered by a doctor in a hospital setting; therefore, a missed dose is not applicable
Side effects

Precautions & warnings
[SAFE] Albumin Human 20% does not usually affect your ability to drive. It is given as an injection into veins under the supervision of a medical professional. Inform your doctor immediately if you experience a skin rash, itching, breathing difficulties, swelling of face and changes in heartbeat while using this medicine. Inform your doctor if you have history of severe anemia (lack of red blood cells), or severe heart failure.
Contraindications
Cardiac failure, severe anaemia, history of hypersensitivity, parenteral nutrition.
Drug interactions
Albumin solution should not be mixed by protein hydrolysates or alcoholic solutions. Risk of atypical reactions to ACE inhibitors in patients undergoing therapeutic plasma exchange with albumin human replacement.
Food interactions

Use in pregnancy

Pediatric use
• Has not been clinically evaluated in children under 12 years of age. • If required, the dose should be determined based on the patient’s clinical condition and body weight. • In neonatal hyperbilirubinemia, a dose of 1 g/kg may be used before exchange transfusion.
Geriatric use
• Use with caution in elderly patients because cardiac function may be reduced. • Monitor for signs of circulatory overload.
Renal / hepatic impairment
[CAUTION] Albumin Human 20% should be used with caution in patients with kidney disease. Dose adjustment of Albumin Human 20% may be needed. Please consult your doctor. [SAFE IF PRESCRIBED] Albumin Human 20% is safe to use in patients with liver disease. No dose adjustment of Albumin Human 20% is recommended.
Overdose effects
• Overdose may cause hypervolemia (circulatory overload), which may present with headache, shortness of breath, increased blood pressure, and pulmonary edema. • At the first signs of circulatory overload, stop the infusion and monitor hemodynamic parameters.
Storage conditions
• Store below 30°C, protected from light. • Do not freeze. • The solution must be clear at the time of use. • Keep out of reach of children.
