Description

Pharmacology
Chloramphenicol inhibits bacterial protein synthesis by binding to 50s subunit of the bacterial ribosome, thus preventing peptide bond formation by peptidyl transferase. It has both bacteriostatic and bactericidal action against H. influenzae, N. meningitidis and S. pneumoniae. Lidocaine is an amide type local anesthetic. It stabilises the neuronal membrane and inhibits Na ion movements, which are necessary for conduction of impulses. In the heart, lidocaine reduces depolarisation of the ventricles during diastole and automaticity in the His-Purkinje system. Duration of action potential and effective refractory period are also reduced.
Mechanism of action
Chloramphenicol stops bacterial growth by inhibiting bacterial protein synthesis. Lidocaine stabilizes the neuronal membrane and blocks pain signal transmission, providing pain relief.
Dosage
Adult Dose (Otic): Otitis externa/Aural otitis: Instill 2-3 drops into the ear 2-3 times daily.
Administration
This medicine is for use in the ear only. Use in the dose and duration as advised by your doctor. Shake well before use.
Missed dose
If you miss a dose of Chloramphenicol 5% + Lidocaine Hydrochloride 1% Ear drops, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

Drug interactions
Decreased effects of iron and vitamin B12 in anemic patients. Phenobarbitone and rifampin reduce efficacy of chloramphenicol. Impairs the action of oral contraceptives. Increases the effect of oral anticoagulants, oral hypoglycemic agents, phenytoin. Avoid concomitant administration with drugs that depress bone marrow function. May increase serum levels with cimetidine and propranolol. Increased risk of cardiac depression with beta-blockers and other antiarrhythmics. Additive cardiac effects with IV phenytoin. Hypokalaemia caused by acetazolamide, loop diuretics and thiazides may antagonise effect of lidocaine. Dose requirements may be increased with long-term use of phenytoin and other enzyme-inducers.
