Description

Citric Acid Monohydrate 5% + Potassium Citrate 30% is an effective alkalinizing agent useful in those conditions where long-term maintenance of alkaline urine is desirable, such as in patients with uric acid and cystine calculi of the urinary tract, especially when the administration of sodium salts is undesirable or contraindicated. Potassium citrate is absorbed and metabolized to potassium bicarbonate and this acts as a systemic alkalizer. This solution is highly concentrated and when administered after meals and before bedtime, allows one to maintain an alkaline urinary pH around the clock, usually without the necessity of a 2 A.M. dose.

Pharmacology

Potassium Citrate is absorbed and metabolized to potassium bicarbonate, this acting as a systemic alkalizer. The effects are essentially those of chlorides before absorption and those of bicarbonates subsequently. Oxidation is virtually complete so that less than 5% of the potassium citrate is excreted in the urine unchanged.

Mechanism of action

Potassium citrate is absorbed and metabolized to potassium bicarbonate and this acts as a systemic alkalizer. It alkalinizes the urine.

Dosage

Adult Dose (UTI discomfort): 10 ml 3 times daily, diluted with 1 glass of water.

Pediatric Dose (1-6 years - UTI): 5 ml 3 times daily, diluted with ½ glass of water.

Pediatric Dose (>6 years - UTI): 10 ml 3 times daily, diluted with 1 glass of water.

Adult Dose (Kidney stone prevention, Gout prevention, Acidosis): 10-15 ml 4 times daily, diluted with 1 glass of water.

Pediatric Dose (Stone prevention, Gout prevention, Acidosis): 5-10 ml 4 times daily, diluted with ½ glass of water.

Administration

Dilute oral solution in water according to directions, followed by additional water. Palatability is enhanced if chilled. Proper dilution may help prevent gastrointestinal injury associated with concentrated potassium salts. Take each dose after meals.

Missed dose

If you miss a dose of this solution, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose. Do not double the dose.

Side effects

This solution is generally well tolerated without any unpleasant side effect when given in recommended doses to patients with normal renal function and urinary output. However, as with any alkalinizing agent, caution must be used in certain patients with abnormal renal mechanisms to avoid development of hyperkalemia or alkalosis.

Potassium intoxication: Listlessness, weakness, mental confusion, tingling of extremities, and other symptoms associated with a high concentration of potassium in the serum.

Precautions & warnings
  • Use with caution in patients with low urinary output.
  • Dilute adequately with water to minimize the possibility of gastrointestinal injury associated with concentrated potassium salts.
  • Preferably take each dose after meals.
  • Large doses may cause hyperkalemia and alkalosis, especially in the presence of renal disease.
  • Do not exceed recommended dosage.
Contraindications
  • Severe renal impairment with oliguria or azotemia
  • Untreated Addison's disease
  • Acute dehydration
  • Severe myocardial damage
  • Hyperkalemia from any cause
Drug interactions

Concurrent administration of potassium-containing medication, potassium-sparing diuretics, angiotensin-converting enzyme (ACE) inhibitors or cardiac glycosides may lead to toxicity.

Food interactions

No food interactions have been found or established.

Use in pregnancy

There are no adequate data from the use of Potassium Citrate oral solution in pregnant women. Should not be used during pregnancy unless clearly necessary and on the advice of a physician.

Use in lactation

It is unknown whether Potassium Citrate is distributed in breast milk. Consult your doctor before use.

Pediatric use

Pediatric dose: 1-6 years: 5 ml 3 times daily; >6 years: 10 ml 3 times daily (for UTI); Stone prevention/Gout/Acidosis: 5-10 ml 4 times daily.

Geriatric use

Limited information is available regarding dose adjustment for elderly patients.

Renal / hepatic impairment

[CAUTION] Use with caution in patients with kidney disease. Contraindicated in severe renal impairment.

Overdose effects

Administration of oral potassium salts to persons with normal excretory mechanisms rarely causes serious hyperkalemia. However, if excretory mechanisms are impaired, hyperkalemia can result. Hyperkalemia, when detected, must be treated immediately because lethal levels can be reached in a few hours.
