Description

Clindamycin 2% Vaginal prep is a combination medicine that is prescribed to treat various types of vaginal infections associated with vaginal discharge. It fights against the infection by stopping the growth of infection-causing microorganisms. It also prevents further spread of the infection. Clindamycin 2% Vaginal prep is to be used as per the label instructions or as suggested by your doctor. This medicine is only for vaginal application. You must wash your hands before and after using it. It is best to use this at bedtime regularly to get the most benefit. If there is any accidental exposure of the medicine to the eyes or mouth, you must wash immediately.

Clindamycin 2% Vaginal prep is a combination of two medicines: Clindamycin and Clotrimazole. Clindamycin is an antibiotic which stops bacterial growth in the vaginal tract by preventing the synthesis of essential proteins required by bacteria to carry out vital functions. Clotrimazole is an antifungal which stops the growth of fungi by preventing them from making their protective covering. Together, they treat your vaginal infection effectively.

Pharmacology

Clindamycin inhibits protein synthesis by reversibly binding to the 50S subunit of the ribosomal, thus blocking the transpeptidation or translocation reactions of susceptible organisms resulting in stunted cell growth. Clotrimazole works by inhibiting the synthesis of ergosterol, an essential component of the fungal cell membrane.

Mechanism of action

Clindamycin binds to the 50S subunit of the ribosomal, inhibiting bacterial protein synthesis. Clotrimazole inhibits fungal cell membrane formation, preventing fungal growth.

Dosage

Adult Dose (Bacterial Vaginosis): Nonpregnant patients: 1 full applicator (approximately 5 grams) intravaginally at bedtime for 3 days. Pregnant patients: 1 full applicator at bedtime for 7 days.

Pediatric Dose (<12 years): Safety and efficacy have not been established.

Administration

This medicine is for external (vaginal) use only. Use it in the dose and duration as advised by your doctor. Check the label for directions before use. Insert into the vagina using the applicator, usually at bedtime. Wash hands before and after use.

Missed dose

If you miss a dose of Clindamycin 2% Vaginal prep, use it as soon as you remember. However, if it is almost time for your next application, skip the missed dose. Do not double the dose.

Precautions & warnings
  • This medicine is inserted into the vagina using an applicator, usually at night before going to bed.
  • Do not skip any doses and finish the full course of treatment even if you feel better.
  • Avoid contact with your eyes. If accidental exposure occurs, rinse immediately with plenty of water.
  • It can damage condoms and diaphragms and make them ineffective at preventing pregnancy. Ask your doctor about other suitable methods of preventing pregnancy.
  • Inform your doctor if your symptoms do not get improved within 3 days of taking the medicine.
  • Contraindicated in individuals with a history of regional enteritis, ulcerative colitis, or a history of "antibiotic-associated" colitis.

Drug interactions

May enhance the action of neuromuscular blocking agents (e.g., atracurium). May antagonise the effects of parasympathomimetics. May competitively inhibit the effects of macrolides, ketolides, streptogramins, linezolid and chloramphenicol. Increased coagulation tests (prothrombin time/INR) and/or bleeding with vitamin K antagonists (e.g., warfarin).

Use in pregnancy

[SAFE IF PRESCRIBED] Clindamycin 2% Vaginal prep is generally considered safe to use during pregnancy. Animal studies have shown low or no adverse effects to the developing baby; however, there are limited human studies. Pregnancy Category B.

Use in lactation

[SAFE IF PRESCRIBED] Clindamycin 2% Vaginal prep is safe to use during breastfeeding. Human studies suggest that the drug does not pass into the breastmilk in a significant amount and is not harmful to the baby.

Pediatric use

Safety and efficacy have not been established in children under 12 years of age.

Geriatric use

Limited information is available regarding dose adjustment for elderly patients.

Renal / hepatic impairment

For topical/vaginal use, no dose adjustment is required in renal or hepatic impairment.

Overdose effects

Excessive application may result in severe irritation. Discontinue use and wait until the skin has recovered. In the event of accidental ingestion, gastrointestinal adverse reactions similar to those seen with systemically administered clindamycin may be seen.
