Description
Diclofenac Sodium inhibits cyclooxygenase (COX)-1 and COX-2, thereby inhibiting prostaglandin synthesis. It may also inhibit neutrophil aggregation/activation, inhibit chemotaxis, decrease proinflammatory cytokine level, and alter lymphocyte activity.

Pharmacology
Diclofenac Sodium inhibits cyclooxygenase (COX)-1 and COX-2, thereby inhibiting prostaglandin synthesis. It may also inhibit neutrophil aggregation/activation, inhibit chemotaxis, decrease proinflammatory cytokine level, and alter lymphocyte activity.
Mechanism of action
Diclofenac Sodium inhibits cyclooxygenase, reducing prostaglandin synthesis, thereby reducing ocular inflammation and pain.
Dosage
Cataract Surgery: 1 drop in affected eye(s) four times daily for 2 weeks, beginning 24 hours post-surgery.
Corneal Refractive Surgery: 1-2 drops within 1 hour pre-op & 15 minutes post-op; continue 1-2 drops four times daily for 3 days.
Inflammatory conditions: 1 drop 4-5 times daily depending upon severity.
Administration
For ophthalmic use only. Hold the dropper close to the eye without touching it. Place the medicine inside the lower eyelid.
Missed dose
If you miss a dose, apply it as soon as possible. However, if it is almost time for your next application, skip the missed dose.
Side effects
Common: Mild ocular stinging, irritation, may slow corneal wound healing, lacrimation, increased IOP, keratitis.
Other: Dizziness, insomnia, pain, fever, chills, nausea/vomiting, abdominal pain, corneal deposits, corneal edema, corneal perforation, corneal thinning.
Precautions & warnings
Diclofenac eye drops may mask signs of infection; be alert to development of infections. During prolonged use, conduct periodic eye examinations including IOP measurement. Contact lenses should not be worn during treatment.
Contraindications
Hypersensitivity; NSAID-induced asthma, urticaria, or acute rhinitis.
Drug interactions
Methotrexate: May increase serum levels.
Other NSAIDs and anticoagulants (warfarin): Higher risk of GI bleeding.
Cyclosporin or triamterene: Increased risk of nephrotoxicity.
Colestyramine and colestipol: Reduce bioavailability.
Sucralfate: Decreased plasma concentration.
Lithium and digoxin: May increase serum levels.
Food interactions
Can be used with or without food.
Use in pregnancy
Pregnancy Category C. Use during pregnancy only if clearly needed.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Excretion in milk significantly lower than systemic administration; possibly compatible.
Pediatric use
Safety and effectiveness in pediatric patients have not been established.
Geriatric use
No specific dosage adjustment is necessary for elderly patients.
Renal / hepatic impairment
No special precautions are necessary for patients with kidney or liver problems.
Overdose effects
Treatment of overdose is symptomatic.
