Description
Empagliflozin + Linagliptin is a combination of empagliflozin an SGLT2 inhibitor and linagliptin a DPP-4 inhibitor. Recommended dose: 10 mg/5 mg once daily in the morning.

Pharmacology
Empagliflozin is an SGLT2 inhibitor that reduces renal glucose reabsorption. Linagliptin is a DPP-4 inhibitor that increases GLP-1 and GIP levels.
Mechanism of action
Empagliflozin inhibits SGLT2 to prevent glucose reabsorption and increase urinary excretion. Linagliptin inhibits DPP-4 to increase incretin hormone levels.
Dosage
10 mg empagliflozin + 5 mg linagliptin once daily in the morning. May be increased to 25 mg/5 mg. eGFR less than 45 mL/min/1.73 m2: Do not initiate.
Administration
Oral tablet. Take once daily in the morning. Swallow whole.
Missed dose
If you miss a dose take it as soon as possible. If it is almost time for your next dose skip the missed dose.
Side effects
Pancreatitis ketoacidosis volume depletion urinary tract infections hypoglycemia Fournier's gangrene genital mycotic infections hypersensitivity reactions severe arthralgia bullous pemphigoid heart failure.
Precautions & warnings
Pancreatitis has been reported with linagliptin. Ketoacidosis has been reported with SGLT2 inhibitors. Empagliflozin can cause volume depletion. SGLT2 inhibitors increase UTI risk.
Contraindications
Severe renal impairment (eGFR less than 45) ESRD or dialysis history of hypersensitivity to linagliptin or empagliflozin.
Drug interactions
Diuretics increase volume depletion risk. Insulin or secretagogues increase hypoglycemia risk. Rifampin decreases linagliptin exposure.
Food interactions
It is not known whether it is safe to consume alcohol.
Use in pregnancy
Not recommended during the second and third trimesters of pregnancy.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Not recommended during breastfeeding.
Pediatric use
Safety and efficacy in pediatric patients not established.
Geriatric use
Limited information available for elderly patients.
Renal / hepatic impairment
eGFR less than 45: Do not initiate; discontinue if eGFR falls below 45. Hepatic: Use with caution.
Overdose effects
