Description
Empagliflozin + Metformin Hydrochloride is a combination of empagliflozin (an SGLT2 inhibitor that reduces renal glucose reabsorption) and metformin hydrochloride (a biguanide that decreases hepatic glucose production and improves insulin sensitivity). Used as an adjunct to diet and exercise in adults with type 2 diabetes. Also indicated to reduce cardiovascular death risk in type 2 diabetes with established cardiovascular disease. Starting dose: empagliflozin 5 mg + metformin 500 mg once daily. Maximum: empagliflozin 25 mg + metformin 2000 mg/day. Take with meals. Contraindicated in type 1 diabetes, severe renal impairment, and lactic acidosis history.

Pharmacology
Empagliflozin is an SGLT2 inhibitor that reduces renal glucose reabsorption and increases urinary glucose excretion. Metformin Hydrochloride is a biguanide that decreases hepatic glucose production and improves insulin sensitivity.
Mechanism of action
Empagliflozin blocks SGLT2 to prevent glucose reabsorption and increase urinary excretion. Metformin decreases hepatic glucose production and increases insulin sensitivity.
Dosage
Adult Dose: Starting dose: empagliflozin 5 mg + metformin 500 mg once daily. Gradually increase to maximum empagliflozin 25 mg + metformin 2000 mg/day.
Renal Dose: eGFR <45 mL/min/1.73 m²: Contraindicated.
Administration
Oral tablet. Take in the dose and duration as advised by your doctor. Swallow whole. Do not chew, crush or break. Take with meals.
Missed dose
If you miss a dose of Empagliflozin + Metformin Hydrochloride, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not take two doses at the same time.

Precautions & warnings
- Empagliflozin + Metformin Hydrochloride is used alone or with other diabetes medicines to control blood sugar.
- It may cause loss of too much fluid (dehydration) or frequent urination. Drink plenty of water.
- May cause hypoglycemia when used with other antidiabetic medicines.
- Can cause genital fungal and/or urinary tract infections; practice good hygiene.
- Monitor blood sugar regularly.
- Inform doctor if you experience constant dizziness, joint pain or unexplained nausea/vomiting.
Contraindications
- Type 1 diabetes
- Diabetic ketoacidosis (DKA)
- Severe kidney disease (eGFR <30 mL/min/1.73 m²)
- Severe liver disease
- Heart failure
- Active bladder infection
- History of lactic acidosis
- Hypersensitivity to empagliflozin or metformin
Drug interactions
Insulin/Sulfonylureas: Increased risk of hypoglycemia. Thiazolidinediones: Increased risk of heart failure. Beta-blockers: May mask hypoglycemia symptoms. ACE inhibitors/ARBs: Increased hypotension risk. Diuretics: Increased dehydration risk. Steroids: Increased hyperglycemia risk.
Food interactions
It is unsafe to consume alcohol with Empagliflozin + Metformin Hydrochloride.
Use in pregnancy
Should not be used during pregnancy.

Use in lactation
Should not be used during breastfeeding.

Renal / hepatic impairment

Overdose effects
Overdose may cause lactic acidosis, hypoglycemia, nausea, vomiting, diarrhea. Hemodialysis is the most effective method to remove lactate and metformin.
