Esomeprazole + Amoxicillin + Clarithromycin Kit is a combination medicine used for the treatment of peptic ulcer disease caused by H. pylori bacteria. It contains two antibiotics and an antacid that eradicates the infection and relieves acid-related symptoms.
Description
This kit contains three medicines: Amoxicillin, Clarithromycin and Esomeprazole. Amoxicillin destroys bacteria by inhibiting cell wall synthesis. Clarithromycin stops bacterial growth by inhibiting protein synthesis. Esomeprazole is a proton pump inhibitor (PPI) that reduces stomach acid production and relieves acid-related indigestion and heartburn.
Indications
Peptic ulcer disease (active chronic gastric, duodenal and gastric ulcers caused by H. pylori infection).

Pharmacology
Esomeprazole irreversibly inhibits H+/K+ ATPase enzyme in gastric parietal cells to suppress gastric acid secretion. Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins. Clarithromycin inhibits protein synthesis by binding to the 50S ribosomal subunit.
Mechanism of action
Amoxicillin inhibits bacterial cell wall synthesis. Clarithromycin inhibits bacterial protein synthesis. Esomeprazole reduces gastric acid production. Together, these three medicines eradicate H. pylori infection and heal ulcers.
Dosage
Twice daily (morning and evening) for 7-14 days. Each strip contains 1 Esomeprazole 20mg capsule, 2 Amoxicillin 500mg tablets and 1 Clarithromycin 500mg tablet.
Administration
Take on an empty stomach. Swallow tablets and capsules whole; do not chew or crush. Take at a fixed time each day.
Missed dose
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not double the dose.
Side effects
Common side effects: Diarrhea, nausea, vomiting, stomach pain, taste change, flatulence, headache, rash. Serious side effects: Allergic reactions (rash, itching, swelling of face, breathing difficulty).
Precautions & warnings
Unsafe during pregnancy. Caution during lactation. Dose adjustment may be needed in kidney disease. Safe in liver disease. Avoid alcohol. Caution while driving.
Contraindications
Hypersensitivity to any component. Allergy to penicillin or macrolide antibiotics.
Drug interactions
Esomeprazole is metabolized through the cytochrome P450 system, especially CYP3A isozymes. Clarithromycin may increase serum theophylline levels. Interactions reported with carbamazepine, cyclosporine, tacrolimus, phenytoin, lovastatin, cisapride, pimozide and astemizole.
Food interactions
Caution advised when consuming alcohol as it may increase drowsiness and dizziness.
Use in pregnancy
Use during pregnancy only if clearly needed. Animal studies have shown harmful effects on the fetus.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Safe during breastfeeding. Does not pass into breast milk in significant amounts.
Pediatric use
Safety and efficacy not established in children.
Geriatric use
No dose adjustment necessary for elderly patients.
Renal / hepatic impairment
Use with caution in kidney disease; dose adjustment may be needed. Safe in liver disease; no dose adjustment needed.
Overdose effects
Limited information on overdose. May cause diarrhea, nausea and stomach pain.
