Glucose 19% + Vamin 18 Novum + Intralipid 20%

Introduction: Glucose 19% + Vamin 18 Novum + Intralipid 20% is a parenteral nutrition solution indicated for patients and children above 2 years of age when oral or enteral nutrition is impossible, insufficient or contraindicated.

How it works: This solution provides glucose (energy source), amino acids (protein synthesis) and lipids (essential fatty acids). Phosphate is utilized for energy production, bone mineralization and DNA synthesis. Electrolytes help maintain fluid balance, nerve function and muscle function.

Important Information: Dose should be individualized based on patient's clinical condition, body weight and nutritional requirements. Infusion rate should not exceed 2.6 ml/kg/hour and should only be infused into a central vein.

Phosphate is efficiently absorbed and utilized by the body for energy production via ATP, bone mineralization, cellular membrane function and DNA synthesis. Glucose serves as a primary source of quick energy. Amino acids support the synthesis of proteins for tissue repair, muscle growth, and immune function. Lipids provide essential fatty acids that are used in membrane synthesis and hormone production.

This parenteral nutrition solution delivers nutrients directly into the bloodstream, providing glucose for energy, amino acids for protein synthesis and lipids for essential fatty acids.

Adults:

• Nitrogen: 0.10-0.15 g/kg/day (normal nutritional state)
• Nitrogen: 0.15-0.30 g/kg/day (moderate to high metabolic stress)
• Total energy: 25-35 kcal/kg/day
• Infusion rate: ≤2.6 ml/kg/hour

Children (2-10 years):

• Start: 12.5-25 ml/kg, increase by 10-15 ml/kg/day
• Maximum: 40 ml/kg/day

Under 2 years: Not recommended

Intravenous (IV) infusion - only into central vein. Infusion period: 12-24 hours.

Common Side Effects:

• Rise in body temperature (<3%)
• Shivering, chills (<1%)
• Nausea/vomiting (<1%)
• Transient increases in liver enzymes
• Thrombophlebitis (if peripheral veins used)
• Hypersensitivity reactions (anaphylactic, skin rash, urticaria)
• Fat overload syndrome

Precautions:

• Monitor fat elimination capacity (serum triglycerides ≤3 mmol/L)
• Correct electrolyte and fluid balance abnormalities before starting
• Central vein use carries infection risk - strict aseptic precautions
• Caution in renal insufficiency, diabetes, pancreatitis, liver impairment

Contraindications:

• Hypersensitivity to egg, soya or peanut protein
• Severe hyperlipaemia
• Severe liver insufficiency
• Severe blood coagulation disorders
• Inborn errors of amino acid metabolism
• Severe renal insufficiency without haemofiltration/dialysis
• Acute shock, acute pulmonary oedema, cardiac failure
• Children under 2 years

Drug Interactions: No significant interactions known.

Food Interactions: No interaction found/established.

Use in Pregnancy: Limited data on use during pregnancy. Use under medical supervision if necessary.

Use in Lactation: Limited data on use during lactation. Consult your doctor.

Pediatric Use: Dosage available for children 2-10 years. Not recommended for children under 2 years.

Geriatric Use: Use as per doctor's advice for elderly patients.

Renal Impairment: Severe renal insufficiency without haemofiltration/dialysis is contraindicated.

Hepatic Impairment: Severe liver insufficiency is contraindicated.

Overdose: Fat overload syndrome (hyperlipaemia, fever, hepatomegaly, anaemia, thrombocytopenia, coma) - discontinue infusion. Hemodialysis may be necessary in severe cases.

Storage: Store at 25°C. Do not freeze.
