Description
Introduction: Homatropine Hydrobromide Eye prep is used before eye examinations and to treat an eye condition called uveitis. It acts by enlarging the pupil of the eyes and helps the doctor visualize the eye more closely.
How it works: Homatropine, a tertiary amine antimuscarinic, produces dilation and loss of accommodation by blocking the response of iris sphincter muscle and the accommodative ciliary muscle to cholinergic stimulation.
Important Information: For refraction: Instill 1-2 drops and repeat 2-3 times at 5-10 minute intervals. For therapy: Instill 1-2 drops every 3-4 hours.

Pharmacology
Homatropine is a competitive muscarinic receptor antagonist with a bulky aromatic group in place of the acetyl group of acetylcholine. It acts in similar manner as atropine, producing similar parasympatholytic effects. By blocking muscarinic receptors and cholinergic signalling pathways, homatropine blocks the response of the iris sphincter muscle and causes mydriasis. It also blocks the accommodative muscle of the ciliary body to cholinergic stimulation.
Mechanism of action
Homatropine blocks muscarinic receptors to relax the iris sphincter muscle, causing pupil dilation and loss of accommodation.
Dosage
Adult Dose:
- For refraction: 1-2 drops of 2% solution, may repeat at 10-15 min intervals (max 5 doses)
- Uveitis: 1-2 drops of 2% bid-tid up to every 3-4 hours as needed
Child Dose:
- Refraction: 1 drop of 2% solution, repeat at 10-min intervals if necessary
- Uveitis (>2 years): 1 drop of 2% solution bid
Under 3 months: Do not use

Administration
For ophthalmic use only. Hold the dropper close to the eye without touching it. Gently squeeze the dropper and place the medicine inside the lower eyelid. Apply pressure on the corner of the eye (close to the nose) for about 1 minute after instilling.
Missed dose
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and follow your regular schedule. Do not double the dose.
Side effects
Common Side Effects:
- Eye irritation
- Stinging sensation
- Hypersensitivity - may cause conjunctivitis
Precautions & warnings
Precautions:
- Do not use in closed-angle glaucoma or narrow angle between iris and cornea
- Caution in prostatic enlargement, paralytic ileus or pyloric stenosis
- Apply pressure on corner of eye for 1 minute after instilling
- Use within 4 weeks of opening

Contraindications:
- Acute haemorrhage
- Narrow-angle glaucoma
- Primary glaucoma
- Predisposition to glaucoma
- Hypersensitivity
Drug interactions
Drug Interactions:
- Effects may be increased by drugs with antimuscarinic effect (some antihistamines, phenothiazines, antipsychotics, TCAs, MAOIs or parasympathomimetics)
Food interactions
Food Interactions: No interaction found/established.
Use in pregnancy
Use in Pregnancy: May be unsafe during pregnancy. Animal studies have shown harmful effects.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Probably safe during breastfeeding. To reduce amount reaching breastmilk, apply pressure over corner of eye for 1 minute and remove excess solution.
Pediatric use
Pediatric Use: Dosage available for children 2 years and older. Do not use in children under 3 months.
Geriatric use
Geriatric Use: Use as per doctor's advice for elderly patients.
Renal / hepatic impairment
Renal & Hepatic Impairment: No interaction found/established (minimal systemic absorption due to topical use).
Overdose effects
Overdose: Overdose is unlikely when used as directed. Excessive use may cause eye irritation.
Storage conditions
Storage: Store in a cool and dry place, protect from light. Do not use longer than 30 days after first opening. Keep out of reach of children.
