Description

Insulin Aspart + Insulin Aspart Protamine is a human insulin analog suspension containing 30% soluble Insulin Aspart and 70% Insulin Aspart Protamine used to lower blood glucose. Insulin Aspart is homologous with regular human insulin except for a single substitution of proline by aspartic acid at position B28. It stimulates peripheral glucose uptake, inhibits hepatic glucose production, inhibits lipolysis and proteolysis, and enhances protein synthesis.

Pharmacology

Insulin Aspart + Insulin Aspart Protamine is a human insulin analog suspension containing 30% Insulin Aspart and 70% Insulin Aspart Protamine. Insulin Aspart is homologous with regular human insulin except for a single substitution of proline by aspartic acid at position B28. The primary activity of insulin is the regulation of glucose metabolism. It stimulates peripheral glucose uptake, inhibits hepatic glucose production, inhibits lipolysis and proteolysis, and enhances protein synthesis.

Mechanism of action

Lowers blood glucose by binding to insulin receptors, facilitating glucose uptake in muscle and fat cells, and inhibiting glucose output from the liver.

Dosage

Adult Dose (Subcutaneous):

  • Typically dosed twice daily (each dose intended to cover 2 meals or a meal and a snack)
  • Individualize and adjust dosage based on metabolic needs, blood glucose monitoring results and glycemic control goal
  • Dosage adjustments may be needed with changes in physical activity, meal patterns, renal or hepatic function

Renal and Hepatic Impairment: Dose reduction may be required

Administration

Administer within 15 minutes before meal initiation as subcutaneous injection in abdominal region, buttocks, thigh, or upper arm; for Type 2 diabetes, may also be given after meal initiation; resuspend thoroughly before use (roll 10 times and move up and down 10 times); rotate injection sites; do not administer IV or in insulin infusion pumps; do not mix with other insulins.

Precautions & warnings

Precautions:

  • Always check insulin labels before administration

Drug interactions

Drug Interactions:

  • Anti-diabetic agents, ACE inhibitors, angiotensin II receptor blockers, disopyramide, fibrates, fluoxetine, MAOIs, pentoxifylline, pramlintide, propoxyphene, salicylates, somatostatin analog, sulfonamide antibiotics decrease insulin requirements
  • Atypical antipsychotics, corticosteroids, danazol, diuretics, estrogen, glucagon, isoniazid, niacin, oral contraceptives, phenothiazine, progestogens, protease inhibitors, somatropin, sympathomimetic agents, thyroid hormones increase insulin requirements
  • Beta-blockers, clonidine, lithium salts, alcohol, pentamidine may either potentiate or weaken insulin effect
  • Beta-blockers, clonidine, guanethidine, reserpine may blunt signs/symptoms of hypoglycemia

Overdose effects

Storage conditions

When not in use: Store in refrigerator (2°C to 8°C); do not freeze; allow to reach room temperature before first use. During use: Not kept in refrigerator; can be kept at room temperature (below 30°C) for up to 4 weeks; keep in outer carton to protect from light; protect from excessive heat and light.
