Description
Loteprednol etabonate is a synthetic nonfluorinated glucocorticoid that stimulates production of lipocortins, proteins that modulate prostaglandin and leukotriene activity. Tobramycin binds to 30S ribosomal subunits, interfering with bacterial protein synthesis. It is active against many aerobic gram-negative bacteria and some aerobic gram-positive bacteria.
Indications
- Steroid-responsive ocular inflammation with risk of superficial bacterial infections
- Post-operative eye inflammation

Pharmacology
Loteprednol stimulates lipocortin production to modulate prostaglandins and leukotrienes; Tobramycin binds to 30S ribosomal subunits to inhibit bacterial protein synthesis.
Mechanism of action
Stops bacterial growth and reduces eye inflammation, redness, and itching.
Dosage
Adult Dose (Eye Drop):
- Instill 1-2 drops into conjunctival sac of affected eye(s) every 4-6 hours
- May use as frequently as every 1-2 hours during first 24-48 hours
- Decrease frequency as signs and symptoms improve
Administration
- For ophthalmic use only
- Wash hands before use
- Hold dropper close to the eye without touching; place drop in the lower eyelid
- Wipe off extra liquid
- To reduce amount reaching breastmilk, place pressure over the corner of the eye for 1 minute

Not generally applicable (regular use); consult doctor if needed.

Precautions & warnings
- Do not wear contact lenses until infection clears up
- Monitor intraocular pressure with prolonged use
- May cause temporary blurred vision; do not drive until vision is clear

Contraindications
- Hypersensitivity to any component or other corticosteroids
- Viral, mycobacterial, and fungal eye infections
Drug interactions
- Enhanced neurotoxic and nephrotoxic effects with other aminoglycosides
- Enhanced toxicity with potent diuretics
- Increased risk of nephrotoxicity with cyclosporin
- Prolonged secondary apnea with neuromuscular blocking agents

Use in pregnancy
Use in Pregnancy: Loteprednol Etabonate 0.5% + Tobramycin 0.3% Eye Prep is unsafe during pregnancy (Category C); definite evidence of risk to the fetus. Use only in life-threatening situations with doctor's advice.

Use in lactation
Use in Lactation: Loteprednol Etabonate 0.5% + Tobramycin 0.3% Eye Prep is probably safe during breastfeeding; limited human data suggests no significant risk to the baby. To reduce amount reaching breastmilk, place pressure over the corner of the eye for 1 minute.
