Description
Micafungin Sodium is a semisynthetic lipopeptide (echinocandin) that inhibits synthesis of 1,3-β-D-glucan in fungal cell wall. It is a sterile lyophilized product for IV infusion.

- Treatment of Candidemia and Acute Disseminated Candidiasis
- Treatment of Esophageal Candidiasis
- Prophylaxis of Candida Infections in Hematopoietic Stem Cell Transplant Patients

Pharmacology
Micafungin Sodium inhibits 1,3-β-D-glucan synthesis in fungal cell wall disrupting cell wall integrity and causing cell death.
Mechanism of action
Prevents fungal growth by inhibiting fungal cell wall formation.
Dosage
Adults and Children ≥4 months:
- Invasive Candidiasis: >40 kg: 100 mg/day; ≤40 kg: 2 mg/kg/day
- Esophageal Candidiasis (≥16 years): >40 kg: 150 mg/day; ≤40 kg: 3 mg/kg/day
- Prophylaxis: >40 kg: 50 mg/day; ≤40 kg: 1 mg/kg/day
Children <4 months:
- Invasive Candidiasis: 4-10 mg/kg/day
- Prophylaxis: 2 mg/kg/day
No loading dose required.
Administration
- For IV infusion only
- Infuse over one hour
- Rapid infusion may cause histamine-mediated reactions
- Do not store reconstituted solution more than 24 hours

Side effects
Common Side Effects:
- Diarrhea
- Nausea and vomiting
- Pyrexia
- Hypokalemia
- Thrombocytopenia
- Headache
Histamine-mediated Reactions:
- Rash and pruritus
- Facial swelling
- Vasodilation
Precautions & warnings
- Discontinue if anaphylaxis occurs
- Monitor for intravascular hemolysis and hemolytic anemia
- Monitor liver function test abnormalities
- Caution in renal dysfunction
Contraindications
- Hypersensitivity to Micafungin or any component
Drug interactions
- Sirolimus Itraconazole Nifedipine: Monitor toxicity; reduce dose if necessary

Use in pregnancy
Use in Pregnancy: No adequate controlled studies. Use only if clearly needed.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Unknown if excreted in breast milk. Caution advised.
Pediatric use
Children <4 months: 4-10 mg/kg/day; ≥4 months: 2 mg/kg/day (invasive)
Geriatric use
No specific dose adjustment for elderly (adult dose applies)
Renal / hepatic impairment
Renal Impairment: Caution in renal dysfunction.
Hepatic Impairment: Monitor liver function tests.
Overdose effects
Micafungin is highly protein bound and not dialyzable. No overdose cases reported. Doses up to 8 mg/kg used in clinical trials.
Storage conditions
Before Reconstitution: Store below 30°C. After Reconstitution: Store below 30°C; use within 24 hours.
