Description
Mitomycin is an antineoplastic antibiotic that is enzymatically reduced to its active metabolite within susceptible cells which causes cross-linking of DNA.

Pharmacology
Mitomycin is enzymatically reduced to an active metabolite within susceptible cells which causes DNA cross-linking.
Mechanism of action
Damages cancer cell DNA to stop their growth and multiplication.
Dosage
Adult Dose:
- Solid Tumors (IV): 4-10 mg (0.06-0.15 mg/kg) every 1-6 weeks
- Combination Therapy: 10 mg/m²
- Intravesical (Bladder Tumor): 20-40 mg weekly or 3 times weekly
Child Dose: Safety and efficacy not established.
Administration
- Administered by doctor or nurse as injection
- Do not self-administer
- For intravesical use, patient should be rotated every 15 minutes
Missed dose
Not applicable (injection/intravesical administration)
Side effects
Common Side Effects:
- Anemia (low red blood cells)
- Decreased white blood cell count
- Increased bleeding tendency
- Loss of appetite
- Nausea and vomiting
- Weight loss
- Skin rash
Serious Side Effects:
- Hemolytic uremic syndrome
- Pulmonary toxicity
- Cardiotoxicity

- Caution while driving
- Caution in renal disease heart disease respiratory disorders
- Increased risk of infection and bleeding
- Contraindicated in pregnancy and lactation
Contraindications
- Hypersensitivity
- Platelet count <100,000/mm³
- Serum creatinine >1.7 mg/dL
- Prolonged prothrombin time
- Pregnancy and lactation
Drug interactions
- Doxorubicin: Increased cardiotoxicity
- Fluorouracil: Intravascular hemolysis and renal failure
- Tamoxifen: Hemolytic uremic syndrome
- Vinca Alkaloids: Acute pulmonary toxicity
Food interactions
Alcohol with Mitomycin may cause excessive drowsiness.
Use in pregnancy
Use in Pregnancy: Highly unsafe. Contraindicated during pregnancy.
Pregnancy Category: D — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Use with caution. Breastfeeding should be held until treatment is completed.
Pediatric use
Safety and efficacy in children not established.
Geriatric use
No specific dose adjustment for elderly but caution advised.
Renal / hepatic impairment
Renal Impairment: Serum creatinine >1.7 mg/dL: Avoid; CrCl <10 mL/min: Reduce dose by 25%.
Hepatic Impairment: Caution and dose adjustment needed in liver disease.
Overdose effects
Overdose may cause bone marrow suppression severe infection and bleeding. Supportive treatment.
