Nateglinide is an anti-diabetic medication used to lower blood glucose levels in type 2 diabetes mellitus. It stimulates insulin release from the pancreas to prevent postprandial hyperglycemia.
Description
Nateglinide is a nonsulfonylurea hypoglycaemic agent that stimulates insulin release from pancreatic β-cells to reduce postprandial hyperglycaemia. This action depends on the amount of existing glucose levels.

Pharmacology
Nateglinide stimulates insulin release from pancreatic β-cells to reduce postprandial hyperglycaemia. It acts in a glucose-dependent manner.
Mechanism of action
Stimulates insulin release to prevent post-meal blood glucose rise.
Dosage
Adult Dose (Oral):
- 60 or 120 mg three times daily (before meals)
Administration
- Take on empty stomach (before meals)
- Swallow tablet whole; do not chew, crush or break
- Take at the same time each day
Missed dose
If you miss a dose, skip it and continue with normal schedule; do not double the dose.
Side effects
Common Side Effects:
- Hypoglycemia (low blood glucose)
- Dizziness
- Increased appetite
- Increased heart rate
- Nausea
- Sweating
- Tiredness
- Weakness
Precautions & warnings
- Carry sugary food or fruit juice for hypoglycemic symptoms (cold sweats, pale skin, tremor, anxiety)
- Increased risk of hypoglycemia with other antidiabetics or alcohol
- Caution while driving
- Monitor blood glucose regularly
Contraindications
- Diabetic ketoacidosis
- Insulin-dependent diabetes mellitus (IDDM)
- Lactation
Drug interactions
- Increased levels/effects with enzyme inhibitors (e.g. fluconazole)
- Increased hypoglycaemic effects with salicylates, MAOIs, nonselective beta-blockers, alcohol, NSAIDs
- Decreased levels/effects with enzyme inducers (e.g. rifampicin)
- Decreased hypoglycaemic effects with thiazide diuretics, corticosteroids, thyroid products, sympathomimetics
Food interactions
Alcohol may increase risk of hypoglycemia; avoid.
Use in pregnancy
Use in Pregnancy: Nateglinide may be unsafe during pregnancy; animal studies show evidence of fetal harm.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Nateglinide is probably safe during breastfeeding; monitor baby's blood glucose.
Pediatric use
Safety and efficacy not established in children.
Geriatric use
Elderly may require dose adjustment.
Renal / hepatic impairment
Renal Impairment: Safe in kidney disease; no dose adjustment needed.
Hepatic Impairment: Caution in liver disease; dose adjustment may be needed.
Overdose effects
Overdose may cause severe hypoglycemia; glucose administration required.
Storage conditions
Store in a dry place, away from light; keep out of reach of children.
