Paracetamol + Phenylephrine + Guaifenesin is a combination used for temporary relief of sinus congestion, headache, minor aches and pains. Paracetamol reduces pain and fever, phenylephrine relieves nasal congestion and guaifenesin loosens phlegm.
Description
Paracetamol blocks pain impulse generation peripherally and may inhibit prostaglandin generation in the CNS; reduces fever by inhibiting hypothalamic heat-regulating center. Phenylephrine is a vasoconstrictor and decongestant that relieves symptoms from upper respiratory tract irritation; shrinks swollen mucous membranes and reduces nasal congestion. Guaifenesin increases respiratory tract fluid secretions and helps to loosen phlegm and bronchial secretions.

Pharmacology
Paracetamol reduces pain and fever by inhibiting prostaglandin generation in the CNS. Phenylephrine stimulates α1-adrenergic receptors to constrict nasal mucosal blood vessels. Guaifenesin stimulates respiratory tract secretory glands to increase fluid secretions and loosen phlegm.
Mechanism of action
Relieves pain, fever, nasal congestion and phlegm symptoms.
Dosage
Adult Dose (Oral):
- 2 tablets every 4 hours; max 12 tablets/24 hours
- Paracetamol: Not to exceed 1g/hr or 4g/day
- Syrup: 30 mL [(325mg/5mg/200mg)/15mL] every 4 hours
Pediatric Dose (>12 years): Same as adult
Administration
- Swallow tablet whole
- Take at the same time each day
- Do not exceed recommended dose
Missed dose
If you miss a dose, take it as soon as you remember; skip if it's almost time for next dose; do not double the dose.
Side effects

- Severe peripheral and visceral vasoconstriction

- Dermatologic rash
- Nausea, vomiting
- Anemia
- Blood dyscrasias (neutropenia, pancytopenia, leukopenia)
- Bilirubin and alkaline phosphatase may increase
Precautions & warnings
- Caution in elderly patients
- Caution in hyperthyroidism, myocardial disease, bradycardia, heart block or severe arteriosclerosis
- Hepatotoxicity possible in chronic alcoholics
- Combined use with other OTC products may cause toxicity
- Risk of serious skin reactions (SJS, TEN, AGEP)
