At a glance
Peritoneal Dialysis Solution (Calcium Chloride + Dextrose + Magnesium Chloride + Sodium Acetate + Sodium Chloride) is a sterile solution used for peritoneal dialysis in patients with end-stage renal disease (ESRD). It helps remove waste products and excess fluid from the body.
Description
Peritoneal Dialysis Solution is a sterile, nonpyrogenic solution used for peritoneal dialysis in patients with end-stage renal disease (ESRD). The solution contains Calcium Chloride, Dextrose Anhydrous, Magnesium Chloride, Sodium Acetate, and Sodium Chloride.
Calcium Chloride prevents and treats negative calcium balance and regulates nerve and muscle function. Dextrose provides calories and water for hydration and reduces protein and nitrogen loss. Sodium Chloride is important for electrolyte and fluid balance and osmotic pressure control. Sodium Acetate is used for fluid and electrolyte replenishment with metabolic alkalinizing ability.
Indications
- Peritoneal Dialysis - End Stage Renal Disease (ESRD)
Therapeutic class
Dialysis Solution
Pharmacological class
Peritoneal Dialysis Fluid
Pharmacology
Calcium Chloride prevents or treats negative calcium balance and regulates action potential excitation threshold to facilitate nerve and muscle function. Sodium Chloride is the major extracellular cation important for electrolyte and fluid balance, osmotic pressure control, and water distribution. Sodium Acetate is used for fluid and electrolyte replenishment with metabolic alkalinizing ability. Dextrose provides calories and water for hydration, reduces protein and nitrogen loss, and promotes glycogen deposition in the liver.
Mechanism of action
When the solution is instilled into the peritoneal cavity, waste products (urea, creatinine) and excess fluid are transferred from the blood to the peritoneal cavity through osmosis and diffusion, which are then drained out.
Dosage
Adults (Intermittent Peritoneal Dialysis - IPD): 2 litres of solution is instilled into the abdominal cavity over a period of 1 hour, then drained from the cavity. The exchange should be continued for 8-36 hours.
Note: Dose, infusion time, and exchange frequency are determined based on the patient's clinical condition and physician's advice.
Administration
For peritoneal dialysis use only. Administered by healthcare professionals during dialysis. Patient should not self-administer.
Missed dose
Hospital-only medicine. Administered by doctor or nurse during peritoneal dialysis. Missed dose concept does not apply (part of the dialysis procedure).
Side effects
Dialysis-related: Loss of amino acids and water-soluble vitamins may occur.
Fluid and electrolyte imbalance: Fluid and electrolyte imbalance, hypovolemia, hypotension, or muscle cramping may occur.
Other: Peritonitis, abdominal pain, infection (catheter infection), hyperglycemia (due to dextrose).
Precautions & warnings
- For peritoneal dialysis use only.
- Caution in patients with illness, infection, or history of abdominal surgery.
- Regular monitoring of electrolytes, blood glucose, and other parameters is required during dialysis.
- Caution/contraindicated in excessive obesity and pregnancy.
Contraindications
- Excessive obesity
- Pregnancy
Drug interactions
- Antibiotics, heparin, insulin: Interactions may occur with peritoneal dialysis solution; follow physician's advice.
