অকাল শিশুদের মধ্যে রেসপ ডিস্ট্রেস সিন্ড্রোম (RDS)। অপরিণত শিশুর মধ্যে প্রতিরোধ <1250 g birthwt বা সার্ফ্যাক্ট্যান্টের ঘাটতির প্রমাণ w/ জন্মের 15 মিনিটের মধ্যে পছন্দ করে। 8 ঘন্টা বয়সের মধ্যে বিশেষভাবে উদ্ধার করুন। 4 mL/kg জন্ম wt (100 mg phospholipids/kg birth wt) 1ম 48 ঘন্টার মধ্যে 4 ডোজ পর্যন্ত ইন্ট্রাট্রাক্যালি। ডোজ 6 ঘন্টার বেশি ঘন ঘন দেওয়া উচিত নয়। যত তাড়াতাড়ি সম্ভব পরিচালনা করা. কেউ জানে না. প্রাকৃতিক বোভাইন পালমোনারি সার্ফ্যাক্ট্যান্ট; শ্বাসযন্ত্রের কষ্ট সিন্ড্রোমে নবজাতকের মধ্যে ঘাটতি অন্তঃসত্ত্বা ফুসফুসের সার্ফ্যাক্ট্যান্ট প্রতিস্থাপন করে। সারফ্যাক্ট্যান্ট অ্যালভিওলার পৃষ্ঠ এবং বাতাসের মধ্যে পৃষ্ঠের উত্তেজনা কমিয়ে মেয়াদ শেষ হওয়ার সময় অ্যালভিওলিকে ভেঙে পড়া থেকে বাধা দেয়। শিশু <600 গ্রাম জন্ম wt. শুধুমাত্র intratracheal ব্যবহারের জন্য। ক্ষণস্থায়ী ব্র্যাডিকার্ডিয়া। O2 ডিস্যাচুরেশন, এন্ডোট্র্যাকিয়াল টিউব রিফ্লাক্স এবং ব্লকেজ, ফ্যাকাশে, ভাসোকনস্ট্রিকশন, হাইপোটেনশন, এইচটিএন, হাইপোকার্বিয়া, হাইপারকার্বিয়া এবং অ্যাপনিয়া।
Indications
Resp distress syndrome (RDS) in premature infants.
Mechanism of action
Natural bovine pulmonary surfactant; replaces deficient endogenous lung surfactant in neonates with respiratory distress syndrome. Surfactant prevents the alveoli from collapsing during expiration by lowering surface tension between alveolar surfaces and air.
Dosage
Child Dose: Prevention in premature infant <1250 g birthwt or w/ evidence of surfactant deficiency Preferably w/in 15 mins of birth. Rescue Preferably by 8 hr of age. 4 mL/kg birth wt(100 mg of phospholipids/kg birth wt) intratracheally up to 4 doses in 1st 48 hr. Doses should not be given more frequently than 6 hrly. To be administered as soon as possible.
Side effects
Transient bradycardia. O2 desaturation, endotracheal tube reflux & blockage, pallor, vasoconstriction, hypotension, HTN, hypocarbia, hypercarbia & apnea.
Precautions & warnings
Infants <600 g birth wt. For intratracheal use only.
