Description
Tenoxicam is an NSAID that inhibits cyclooxygenase enzymes, preventing prostaglandin synthesis. It inhibits leukocyte function (phagocytosis and chemotaxis), scavenges free oxygen radicals, and inhibits human metalloproteinases to reduce cartilage breakdown, exhibiting anti-inflammatory, analgesic, and antipyretic activity.

Pharmacology
Tenoxicam inhibits COX-1 and COX-2 enzymes, preventing prostaglandin synthesis. It inhibits leukocyte phagocytosis and chemotaxis, scavenges free oxygen radicals, and inhibits human metalloproteinases to reduce cartilage breakdown, providing anti-inflammatory, analgesic, and antipyretic effects.
Mechanism of action
Reduces inflammation and pain by inhibiting prostaglandin synthesis.
Dosage
Adult Dose (Oral):
- Standard: 20 mg/day (at the same time each day)
- Acute gout: 40 mg/day for 2 days, then 20 mg/day for 5 days
- Chronic disorders: Not to exceed 20 mg/day; maintenance 10 mg/day
- Elderly: Use the lowest effective dose
Administration
- Take with or immediately after a meal
- Take with a glass of water
- Take at the same time each day
Missed dose
If you miss a dose, take it as soon as you remember; if almost time for the next dose, skip it; do not double the dose.
Side effects
Common Side Effects (>1%):
- Gastric, epigastric, and abdominal discomfort, dyspepsia, heartburn, nausea
- Dizziness, headache
Rare (<1%):
- Constipation, diarrhea, stomatitis, vomiting, ulcers, GI bleeding
- Fatigue, sleep disturbances, dry mouth, vertigo
- Skin rash, pruritus, photosensitivity
- Elevated liver enzymes, renal function changes
Precautions & warnings
- Do not use in active or history of recurrent GI bleeding or peptic ulcer
- Do not use in severe heart failure, hemorrhagic diathesis, asthma
- Caution in renal or hepatic impairment
- Do not use in the 3rd trimester of pregnancy
Contraindications
- Hypersensitivity to NSAIDs or salicylates
- Active or recurrent GI bleeding/peptic ulcer
- Severe heart failure, hemorrhagic diathesis
- 3rd trimester of pregnancy
Drug interactions
- Salicylates and other NSAIDs: Increased GI adverse reactions (avoid)
- Warfarin: Enhanced anticoagulant effect
- Methotrexate: Increased toxicity
- Lithium: Decreased renal clearance and toxicity
- Diuretics: Interfere with natriuretic action
- Corticosteroids: Increased GI bleeding risk
Food interactions
Alcohol is unsafe with Tenoxicam (increased GI bleeding risk).
Use in pregnancy
Use in Pregnancy: May be unsafe; animal studies show harmful effects; do not use in the 3rd trimester; consult your doctor.
Pregnancy Category: D — always consult a doctor before taking during pregnancy.
