Description
Tobramycin is an aminoglycoside antibiotic that binds to the 30S ribosomal subunit, interfering with bacterial protein synthesis. Inhaled tobramycin reaches high concentrations in the lungs and is effective against Gram-negative bacteria, including Pseudomonas aeruginosa.

Pharmacology
Tobramycin binds to the 30S ribosomal subunit, inhibiting bacterial protein synthesis. Inhaled tobramycin achieves high concentrations in the lungs, making it effective against Pseudomonas aeruginosa.

Dosage
Adult Dose (Inhalation):
- 300 mg ampoule every 12 hours inhaled from a suitable nebulizer for 28 days, then stop for 28 days; repeat in cycles of 28 days as needed
Pediatric Dose (>6 years):
- Same as adult dose

- For inhalation only (via nebulizer)
- Use in a hospital or at home as instructed
- Clean and maintain the nebulizer

- Voice alteration (12.8%)

- Myalgia (4.7%)

Precautions & warnings
- Caution in pre-existing renal and auditory/vestibular impairment
- Caution in dehydration, neuromuscular blockade, myasthenia gravis, parkinsonism
- Caution in neonates, elderly, and lactation

Contraindications
- Hypersensitivity to aminoglycosides
- Pregnancy
Drug interactions
- Etacrynic acid, furosemide, other aminoglycosides, cefalotin, cisplatin, vancomycin, piperacillin, cyclosporine, amphotericin B: Increased nephrotoxicity or ototoxicity
- Miconazole: Decreased serum tobramycin levels
- Magnesium and other neuromuscular-blocking agents: Increased neuromuscular-blocking activity (potentially fatal)
Food interactions
Alcohol consumption is not relevant (inhalation).
