Description
Varenicline Tartrate is a prescription medicine used to quit smoking by reducing the craving for a cigarette and the irritable feeling that occurs when you stop smoking. It works by blocking nicotine effect on the brain and this suppresses enjoyment while smoking or the urge to smoke. Varenicline Tartrate may be taken with or without food, preferably at the same time each day. Take it in the dose and duration as advised by your doctor. If you have missed a dose, take it as soon as you remember. Do not skip any doses and finish the full course of treatment. You should never stop this medicine suddenly without talking to your doctor. It is important that this medication is not stopped suddenly without talking to your doctor. Some common side effects of using this medication are nausea, headache, insomnia (difficulty sleeping), abnormal dreams and nasopharyngitis (inflammation of the throat and nasal passages). Initially, it may also cause dizziness and sleepiness, so do not drive or do anything that requires mental focus until you know how this medicine affects you. Before taking Varenicline Tartrate, inform your doctor if you have a history of heart diseases, fits (seizures), or other mental disorders. Remember it will take time, support, willpower and determination to quit the habit of smoking. It should be used as a smoking cessation aid along with programs including education, counseling, and support. Quitting smoking reduces the risk of developing further health related problems such as heart or lung diseases as well as certain types of cancer. Varenicline Tartrate is a smoking cessation aid. It works by mimicking the effect of nicotine by binding to the nicotine receptors in the brain. This reduces the urge to smoke, relieves withdrawal symptoms, and helps you quit smoking.

Mechanism of action
Varenicline Tartrate is a partial neuronal alpha 2 beta 2 nicotinic acetylcholine receptor agonist. It works as a smoking cessation aid by preventing nicotine stimulation of mesolimbic dopamine system. It also stimulates dopamine activity but to a much lesser extent than nicotine does, resulting in reduced craving and withdrawal symptoms.
Dosage
Adult Dose: Smoking Cessation Set a date to stop smoking and begin varenicline 1 week before this date Alternatively, the patient can begin varenicline and then quit smoking between days 8 and 35 of treatment Also see Administration Quit smoking date regimen Initiate regimen 1 week before quit smoking date Days 1-3: 0.5 mg PO qDay Days 4-7: 0.5 mg PO BID Day 8 to end of treatment: 1 mg PO BID If quitting is successful after 12 weeks, continue another 12 weeks at 1 mg q12hr Gradual approach to quitting For patients who are sure that they are not able or willing to quit abruptly, consider a gradual approach to quitting smoking with varenicline Begin dosing and reduce smoking by 50% from baseline within the first 4 weeks, by an additional 50% in the next 4 weeks, and continue reducing with the goal of reaching complete abstinence by 12 weeks Continue varenicline for an additional 12 weeks, for a total of 24 weeks of treatment Encourage patients to attempt quitting sooner if they feel ready Child Dose: <18 years: Safety and efficacy not established Renal Dose: Renal impairment Mild-to-moderate (CrCl ?30 mL/min): No dosage adjustment required Severe (CrCl <30 mL/min): 0.5 mg PO qDay initially; may increase to 0.5 mg PO q12hr ESRD on hemodialysis: Not to exceed 0.5 mg PO qDay
Administration
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Varenicline Tartrate may be taken with or without food, but it is better to take it at a fixed time.
Missed dose
If you miss a dose of Varenicline Tartrate, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.
Side effects

Precautions & warnings

In addition, counselling should be done to increase the chances of a successful quit attempt. Quitting smoking can lower your chances of having lung disease, heart disease or getting certain types of cancer that are related to smoking. Varenicline Tartrate can make you very sleepy. Don't drive or do anything requiring concentration until you know how it affects you. Stop taking Varenicline Tartrate and inform your doctor if you notice sudden mood and behavior changes or develop suicidal thoughts. Inform your doctor if you have a history of heart diseases, fits (seizures), or other mental disorders.
Contraindications
Documented hypersensitivity or skin reactions to drug or components of formulation. Nonsmokers.
Drug interactions
Increased incidence of adverse effects (nausea, vomiting) w/ transdermal nicotine. Increased exposure w/ cimetidine.
Food interactions

Use in pregnancy

Use in lactation
[CAUTION] Varenicline Tartrate should be used with caution during breastfeeding. Breastfeeding should be held until the treatment of the mother is completed and the drug is eliminated from her body.
Renal / hepatic impairment
[CAUTION] Varenicline Tartrate should be used with caution in patients with kidney disease. Dose adjustment of Varenicline Tartrate may be needed. Please consult your doctor.
