Description

The standard triple therapy regimen contains 20 mg vonoprazan tablet, 500 mg amoxicillin capsule and 500 mg clarithromycin tablet for oral administration is effective for eradicating H. pylori.

Pharmacology

Vonoprazan inhibits H+, K+-ATPase in gastric parietal cells, blocking gastric acid production. Amoxicillin is an antibacterial drug. Clarithromycin is a macrolide antimicrobial drug.

Mechanism of action

Vonoprazan blocks gastric acid production, and amoxicillin and clarithromycin destroy H. pylori bacteria.

Dosage

Adult Dose (Oral):

  • Vonoprazan 20 mg, Amoxicillin 1000 mg, Clarithromycin 500 mg
  • Each given twice daily (morning and evening, 12 hours apart), with or without food, for 14 days
Administration
  • May be taken with or without food
  • Use as directed by a physician
Missed dose

If you miss a dose, take it as soon as you remember; if almost time for next dose, skip it; do not double the dose.

Side effects

Common Side Effects:

  • Dysgeusia
  • Diarrhea
  • Vulvovaginal candidiasis
  • Headache
  • Abdominal pain
  • Hypertension
  • Nasopharyngitis
Precautions & warnings
  • Serious and occasionally fatal hypersensitivity reactions (e.g., anaphylaxis) have been reported
  • Discontinue at the first signs or symptoms of SCAR
  • Evaluate if diarrhea occurs with this pack
  • Avoid in patients with known QT prolongation
  • Discontinue if signs and symptoms of hepatitis occur
Contraindications
  • Known hypersensitivity to vonoprazan, amoxicillin or any other betalactams, clarithromycin or any other macrolide antimicrobials
Drug interactions
  • Tacrolimus, cyclosporine, clopidogrel, citalopram, cilostazol, probenecid, allopurinol, itraconazole, ketoconazole, erlotinib, dasatinib, nilotinib, mycophenolate mofetil, saquinavir, ritonavir, etravirine, rilpivirine-containing products, atazanavir, nelfinavir
Food interactions

Consuming alcohol while taking this triple pack may cause symptoms such as flushing, increased heart beat, nausea, thirst, chest pain and low blood pressure (Disulfiram reaction).

Use in pregnancy

Use in Pregnancy: Based on findings from animal studies and observational studies in pregnant women with use of clarithromycin, use of this triple pack is not recommended in pregnant women.

Pregnancy Category: C — always consult a doctor before taking during pregnancy.

Use in lactation

Use in Lactation: There are no data regarding the presence of vonoprazan in human milk; vonoprazan and its metabolites are present in rat milk.

Pediatric use

Safety and effectiveness not established in pediatric patients

Geriatric use

Caution required in elderly; increased risk of torsade de pointes due to the clarithromycin component

Renal / hepatic impairment

Renal Impairment: Avoid use in severe renal impairment

Hepatic Impairment: Avoid use in moderate and severe hepatic impairment

Overdose effects

Symptoms: Limited information.

Treatment: Consult with doctor immediately.

Do not store above 30°C. Keep in a dry place. Protect from light and keep out of the reach of children.
