Description

Albendazole is a broad-spectrum anthelmintic effective against intestinal nematodes, cestodes, and tissue helminths. It exerts larvicidal, ovicidal, and vermicidal activities by inhibiting tubulin polymerization in parasites.

Indications

Indicated in infestations of Hookworm, Roundworm, Threadworm, Whipworm, Strongyloides, Tapeworm, Opisthorchis, Giardiasis, and Hydatid disease.

Pharmacology

Albendazole blocks glucose uptake in susceptible helminths, depleting their energy levels. It inhibits parasite tubulin polymerization. It undergoes rapid first-pass metabolism to the active metabolite albendazole sulphoxide (half-life ~8.5 hours).

Mechanism of action

Binds to parasite tubulin to inhibit microtubule polymerization and disrupts glucose absorption, causing energy depletion and parasite death.

Dosage

Adults & Children >2 years: 400 mg as a single dose for common worms. For strongyloidiasis or taeniasis: 400 mg daily for 3 consecutive days. Children 1-2 years: 200 mg single dose. Hydatid disease (>60 kg): 400 mg twice daily for 28 days.

Administration

For oral use. Tablets may be chewed, swallowed with water, or crushed. Administration with fatty meals increases systemic absorption.

Missed dose

Take as soon as remembered. Skip if it is close to the next scheduled dose. Do not take a double dose.

Side effects

Gastrointestinal discomfort, headache, dizziness, elevated liver enzymes, reversible alopecia, and rarely blood disorders (leukopenia, pancytopenia).

Precautions & warnings

Perform blood counts and liver function tests before and during treatment cycles in Hydatid disease. Exclude pregnancy prior to initiating therapy.

Contraindications

Contraindicated during pregnancy or in women suspected to be pregnant, and in patients with known hypersensitivity to benzimidazoles.

Drug interactions

Plasma levels of the active metabolite may be increased by dexamethasone, praziquantel, or cimetidine.

Food interactions

Systemic absorption is significantly increased when taken with fatty foods.

Use in pregnancy

FDA Pregnancy Category C. Contraindicated in pregnancy and women planning pregnancy.

Pregnancy Category: C — always consult a doctor before taking during pregnancy.

Use in lactation

Excretion in human milk is unknown. Exercise caution when administering to nursing mothers.

Pediatric use

Not recommended in children under 1 year. Dosage is reduced to 200 mg for children aged 1–2 years.

Geriatric use

No dosage adjustment needed, but use with standard precautions.

Renal / hepatic impairment

No dose adjustment required in renal or hepatic failure, but monitor LFTs during prolonged therapy.

Overdose effects

Symptomatic treatment and supportive care should be initiated upon accidental overdose.

Storage conditions

Store in a dry place below 30°C, protected from light and heat. Keep out of reach of children.
