Secukinumab is a fully human IgG1/κ monoclonal antibody that selectively inhibits IL-17A. It is used for the treatment of moderate-to-severe plaque psoriasis, active psoriatic arthritis, and active ankylosing spondylitis.
Indications
- Moderate-to-severe plaque psoriasis (candidates for systemic therapy or phototherapy)
- Moderate-to-severe scalp psoriasis
- Active psoriatic arthritis (PsA)
- Active ankylosing spondylitis (AS)
Dosage
Adult Dose (Subcutaneous):
- Plaque psoriasis: 300 mg at Weeks 0,1,2,3,4; then 300 mg every 4 weeks (150 mg may be acceptable)
- Psoriatic arthritis (with coexistent plaque psoriasis): Same as above
- Psoriatic arthritis (others): 150 mg at Weeks 0,1,2,3,4; then every 4 weeks; increase to 300 mg if inadequate response
- Ankylosing spondylitis: 150 mg at Weeks 0,1,2,3,4; then every 4 weeks

Common Side Effects (>10%):
- Infections (28.7%)
- Nasopharyngitis (11.4-12.3%)
1-10%:
- Diarrhea (2.6-4.1%)
- URT infection (2.5-3.2%)
- Rhinitis (1.4%)
- Oral herpes (0.1-1.3%)
- Pharyngitis (1-1.2%)
- Urticaria (0.6-1.2%)
Precautions & warnings
- Do not use in serious infections (active TB, hepatitis B, sepsis)
- Caution in history of chronic or recurrent infections
- Caution in inflammatory bowel disease (Crohn's disease)
- Do not use with live vaccines
- Caution in pregnancy and lactation
