Memantine + Donepezil is used for the treatment of moderate to severe dementia of the Alzheimer's type. Memantine acts as an NMDA receptor antagonist and Donepezil acts as an acetylcholinesterase inhibitor.
Indications
- Moderate to severe dementia of the Alzheimer's type (patients stabilized on donepezil 10 mg once daily)
Dosage
Adult Dose (Oral):
- On donepezil 10 mg only: Initial 7/10 mg in the evening; increase by 7 mg weekly to maintenance 28/10 mg
- On memantine (28 mg ER) + donepezil 10 mg: 28/10 mg in the evening
- Severe renal impairment: Maintenance 14/10 mg in the evening
Side effects
Common Side Effects (≥5%):
- Headache
- Diarrhea
- Dizziness
- Anorexia
- Nausea, vomiting
- Ecchymosis
Precautions & warnings
- May exaggerate succinylcholine-type muscle relaxation during anesthesia
- Vagotonic effects on sinoatrial and atrioventricular nodes (bradycardia or heart block)
- Monitor for gastrointestinal bleeding
- May cause bladder outflow obstruction
- Elevated urine pH decreases memantine elimination
