Sotagliflozin is an SGLT1 and SGLT2 inhibitor used as an adjunct to insulin therapy to improve glycemic control in type 1 diabetes mellitus (BMI ≥27 kg/m² with inadequate glycemic control despite optimal insulin therapy).

Indications
  • Type 1 diabetes mellitus (BMI ≥27 kg/m², inadequate glycemic control despite optimal insulin therapy)
Dosage

Adult Dose (Oral):

  • Recommended: 200 mg once daily (before the first meal of the day)
  • If additional glycemic control is needed: Increase to 400 mg once daily after at least 3 months
Side effects

Side Effects:

  • Hypotension
  • Renal impairment
  • Hepatic impairment
  • Genital mycotic infections
  • Urinary tract infections
  • Nausea, vomiting, abdominal pain
  • Excessive thirst, constant fatigue
  • Ketonuria or elevated beta-hydroxybutyrate
  • Rapid weight loss
Precautions & warnings
  • Risk of hypoglycemia with insulin (may require insulin dose reduction)
  • Hypotension: Assess volume status in elderly, renal impairment, low systolic BP, patients on diuretics
  • eGFR <60: Not recommended to initiate; discontinue if eGFR <45
  • Do not use in moderate-severe hepatic impairment
