Sotagliflozin is an SGLT1 and SGLT2 inhibitor used as an adjunct to insulin therapy to improve glycemic control in type 1 diabetes mellitus (BMI ≥27 kg/m² with inadequate glycemic control despite optimal insulin therapy).
Indications
- Type 1 diabetes mellitus (BMI ≥27 kg/m², inadequate glycemic control despite optimal insulin therapy)
Dosage
Adult Dose (Oral):
- Recommended: 200 mg once daily (before the first meal of the day)
- If additional glycemic control is needed: Increase to 400 mg once daily after at least 3 months
Side effects
Side Effects:
- Hypotension
- Renal impairment
- Hepatic impairment
- Genital mycotic infections
- Urinary tract infections
- Nausea, vomiting, abdominal pain
- Excessive thirst, constant fatigue
- Ketonuria or elevated beta-hydroxybutyrate
- Rapid weight loss
Precautions & warnings
- Risk of hypoglycemia with insulin (may require insulin dose reduction)
- Hypotension: Assess volume status in elderly, renal impairment, low systolic BP, patients on diuretics
- eGFR <60: Not recommended to initiate; discontinue if eGFR <45
- Do not use in moderate-severe hepatic impairment
