Description
Busulphan is used in the treatment of some cancers of blood like chronic myeloid leukaemia. It is used in combination with other cancer medicines during hematopoietic progenitor cell transplantation (HPCT). It may also be used to treat some other types of cancer. Busulphan can be taken with or without food, but better to take it at the same time every day. Your doctor will decide what dose is necessary and how often you need to take it. This will depend on what you are being treated for and may change from time to time. You should take it exactly as your doctor has advised. Taking it in the wrong way or taking too much can cause very serious side effects. It may take several weeks or months for you to see or feel the benefits but do not stop taking it unless your doctor tells you to. The most common side effects of this medicine include decreased blood cells (red cells, white cells, and platelets), nausea, vomiting, and loss of appetite. If these bother you or appear serious, let your doctor know. There may be ways of reducing or preventing them. Inform your doctor if you have a medical history, of bleeding disorders, heart or liver disease, radiation treatment or any infection. Many other medicines can affect, or be affected by, this medicine so let your doctor know all medications you are using. Inform your doctor immediately if you notice unusual bruising or bleeding or if you have dark, tarry, or bloody stools and sore throat. This medicine is known to reduce the number of blood cells in your blood thereby, increasing the susceptibility to infections. Regular blood tests are required to check your blood cells along with kidney, liver and heart function during treatment with this medicine. The use of effective contraception by both males and females during treatment is important to avoid pregnancy. Busulphan is an anti-cancer medication. It works by damaging the genetic material (DNA and RNA) of the cancer cells. This stops their growth and multiplication.
Indications
• Blood cancer (Chronic myeloid leukaemia)
Mechanism of action
Busulfan reacts with N-7 position of guanosine and interferes with DNA replication and RNA transcription by alkylating and cross-linking the DNA strands.
Dosage
Adult Dose: Oral Palliative treatment of chronic myeloid leukemia Adult: 60 mcg/kg daily. Maintenance: 0.5-2 mg daily. Max: 4 mg daily. Polycythemia vera Adult: 4-6 mg daily continued for 4-6 wk with blood counts monitoring. Essential thrombocythemia Adult: 2-4 mg daily. Conditioning regimens for bone marrow transplantation Adult: 3.5-4 mg/kg daily in divided doses for 4 days up to a total dose of 14-16 mg/kg. Usually used with cyclophosphamide for ablation of recipient's bone marrow.
Administration
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Busulphan may be taken with or without food, but it is better to take it at a fixed time.
Missed dose
If you miss a dose of Busulphan, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.
Side effects
Common: • Decreased blood cells (red cells, white cells, and platelets) • Abdominal pain • Anxiety • Nausea • Vomiting • Loss of appetite • Decreased magnesium level in blood • Decreased potassium level in blood • Diarrhea • Fever • Headache • Increased glucose level in blood • Insomnia (difficulty in sleeping) • Stomatitis (Inflammation of the mouth)
Precautions & warnings
[CONSULT YOUR DOCTOR] It is not known whether Busulphan alters the ability to drive. Do not drive if you experience any symptoms that affect your ability to concentrate and react. Seek immediate medical attention if you have unusual bruising or bleeding, or signs of infection (fever, chills, body aches) as busulfan can lower the ability of your blood to clot and body to fight infections. Inform your doctor if you have thalassemia (abnormal hemoglobin in blood), gout (arthritis due to impaired metabolism of uric acid) or if you have had radiotherapy. Do not receive live vaccine while using busulfan, and avoid contact with individuals who have recently received the same. Tell your doctor if you are or planning to become pregnant or are breastfeeding. Common Side Effects.
Contraindications
Pregnancy and lactation. Hypersensitivity.
Drug interactions
Decreased clearance when used with cyclophosphamide and itraconazole. Increased clearance by phenytoin. May reduce response to vaccines, possibility of generalized infections with live vaccines. Combination with thioguanine results in oesophageal varices and abdominal liver function tests. Potentially Fatal: Cytotoxic agents may increase risk of pulmonary toxicity.
Food interactions

Use in pregnancy

Use in lactation
[UNSAFE] Busulphan is unsafe to use during breastfeeding. Data suggests that the drug may cause toxicity to the baby.
Renal / hepatic impairment
[SAFE IF PRESCRIBED] Busulphan is probably safe to use in patients with kidney disease. Limited data available suggests that dose adjustment of Busulphan may not be needed in these patients.
