Nicorandil is a medicine used to prevent and treat heart-related chest pain (angina). It is usually prescribed when other heart medicines are not suitable or have not worked. It reduces the risk of an angina attack by widening blood vessels and improving blood flow to the heart. Nicorandil may be taken with or without food. It should be swallowed whole with water at the same time(s) each day. Your doctor will decide the dose and how often you should take it. This may change from time to time depending on how well it is working. You should take this medicine regularly to get the maximum benefit, even if you feel fine. Doing so will help you prevent any further harm. If you stop taking it, your chest pain may come back. You can improve the health of your heart by making some lifestyle changes such as quitting smoking, cutting down on alcohol, eating well, exercising regularly and managing stress. The most common side effect of this medicine is headache, especially during the first few days of treatment. You can reduce this by drinking plenty of fluids and avoiding alcohol, but it usually goes after a week. Other common side effects include feeling dizzy or weak, nausea and flushing (sense of warmth in the face, ears, neck and trunk). Talk to your doctor if these bother you or do not go away. It is not suitable for some people. Tell your doctor if you have low blood pressure or heart failure. Also let your doctor know all other medications you are using as some may affect, or be affected by this medicine. You should not take any medicines for erectile dysfunction with this medicine. This medicine is not usually recommended during pregnancy or breastfeeding. Avoid drinking alcohol, as it can increase the blood pressure-lowering effect of this medicine. Your doctor may ask you to avoid salt substitutes. Nicorandil is a vasodilator. It works by reducing the workload of the heart. As a result, heart muscle does not need as much blood and oxygen supply. This reduces the frequency and severity of angina attacks.

• Angina (heart-related chest pain)

Nicorandil dilates arterioles and large coronary arteries by opening the potassium channels, and stimulates guanylate cyclase causing venous vasodilatation. It therefore reduces preload and afterload, and improves coronary blood flow.

Adult Dose: Oral Angina pectoris Adult: 10 mg bid, increase if necessary. Usual dose: 10-20 mg bid. May use 5 mg bid for patients prone to headache. Max dose: 30 mg bid. Hepatic impairment: Dose reduction may be necessary.

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Nicorandil may be taken with or without food, but it is better to take it at a fixed time.

If you miss a dose of Nicorandil, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

[UNSAFE] Nicorandil may decrease alertness, affect your vision or make you feel sleepy and dizzy. Do not drive if these symptoms occur. You must continue to take it as advised, usually for a long time. Nicorandil may cause headaches. Drink plenty of water and take a suitable painkiller. Inform your doctor if it does not go away. Nicorandil may cause dizziness or sleepiness. Do not drive or do anything which requires concentration until you know how it affects you. Do not consume alcohol while taking Nicorandil as that may cause increased dizziness. Do not take medications like sildenafil or tadalafil (prescribed for erectile dysfunction) while on treatment with Nicorandil as it may cause low blood pressure. Inform your doctor if you notice any bleeding from the anal passage, blood in vomit or black stools.

Cardiogenic shock; hypotension; left ventricular failure with low filling pressure; lactation.

Hypotensive interaction may occur with alcohol, TCAs, antihypertensives and other vasodilators. Potentially Fatal: Enhanced hypotensive effect with sildenafil and other phosphodiesterase type-5 inhibitors.

I can understand you had a heart attack earlier & Angioplasty must have been done. So i will advice you for a Stress Echo to rule out cardiac cause of this pain. You have to stop Embeta 2 days prior to stress echo. I will also advice you to start Tab Nikoran 5mg twice daily, but stop this too 2 days prior to the test.
