Description
Nortriptyline is known as a tricyclic antidepressant. It is used to treat depression and night time bedwetting (nocturnal enuresis) by older children at different doses. Additionally, it may also be prescribed for neuropathic pain. Nortriptyline works by increasing the levels of chemical messengers in the brain that stabilizes and enhances the mood. It is better to take it before bedtime because it can make you feel drowsy. It can be taken with or without food, but you should take it at a fixed time each day for better efficacy. The dose and duration will be decided by your doctor so that you get the right amount to control your symptoms. The most common side effects of this medicine include increased heart rate, blurred vision, dryness in the mouth, difficulty in urination and constipation. Initially, this medicine may also cause a sudden drop in the blood pressure when you change positions. It may even cause dizziness and sleepiness, so do not drive or do anything that requires mental focus until you know how this medicine affects you. To lower the chance of dizziness, rise slowly if you have been sitting or lying down. It may also lead to weight gain in some people. Most of the common side effects tend to be mild and temporary. Your doctor may be able to suggest ways of preventing or reducing side effects if they bother you or do not go away. Serious side effects associated with this medicine are rare. Before taking this medicine, it is important to tell your doctor if you are taking or have recently taken any other medicines for the same or any other diseases. Pregnant and breastfeeding women should take this medicine with proper consultation and caution. This medicine is not known to be addictive, but you can experience additional side effects (withdrawal symptoms) if you stop taking it suddenly. If you notice any sudden mood change or get suicidal or self-harm thoughts, you must consult the doctor without delay. Consumption of alcohol should be avoided as alcohol interacts with this medicine to cause serious health issues. Nortriptyline is a tricyclic antidepressant. Mechanism of action
Nortriptyline, a dibenzocycloheptadiene tricyclic antidepressant, is the primary active metabolite of amitriptyline. It increases synaptic concentration of serotonin and/or norepinephrine in the CNS by blocking the neuronal reuptake of norepinephrine and serotonin.
Dosage
Adult Dose: Oral Depression Adult: 75-100 mg/day in 3 or 4 divided doses. Max: 150 mg/day in severe depression. Elderly: 30-50 mg/day in divided doses. Hepatic impairment: Administer lower dose and titrate at a slower rate Child Dose: Oral Depression Child: Adolescent: 30-50 mg/day in divided doses. Nocturnal enuresis Child: 6-7 yr (20-25 kg): 10 mg; 8-11 yr (25-35kg): 10-20 mg; >11 yr (35-54 kg): 25-35 mg. All doses are given 30 minutes before bedtime and treatment should continue for not >3 mth.
Administration
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Nortriptyline may be taken with or without food, but it is better to take it at a fixed time.
Missed dose
If you miss a dose of Nortriptyline, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.
Side effects
Common: • Increased heart rate • Weight gain • Difficulty in urination • Orthostatic hypotension (sudden lowering of blood pressure on standing) • Dryness in mouth • Constipation
Precautions & warnings
[UNSAFE] Nortriptyline may cause side effects which could affect your ability to drive. As Nortriptyline may affect your alertness which may affect your ability to drive. Nortriptyline is used for the treatment of depression and bedwetting in children. The main dose should be taken at night as this drug may cause drowsiness or sleepiness. You may be asked for regular blood tests to monitor the blood cell count or level of sodium and sugar in the blood. Inform your doctor if you notice behavioral changes, mood changes and suicidal thoughts while taking this medicine. Do not drive or do anything that requires concentration until you know how the medicine affects you. Do not stop taking the medication suddenly without talking to your doctor.
Contraindications
Mania, recent MI, arrhythmias (particularly heart block); severe liver disease; children <6 yr.
Drug interactions
Increased risk of serotonin syndrome w/ SSRIs, SNRIs, TCAs, triptans, fentanyl, lithium, tramadol, tryptophan, buspirone. May reduce antihypertensive effect of bethanidine, guanethidine, debrisoquine, clonidine. May increase metabolism w/ barbiturates. Increased risk of arrhythmias and hypotension w/ anaesth. Increased plasma level w/ fluoxetine. Potentially Fatal: Increased risk of serotonin syndrome w/ MAOIs, linezolid and methylene blue.
Food interactions
[UNSAFE] Nortriptyline may cause excessive drowsiness with alcohol.
Use in pregnancy
[CONSULT YOUR DOCTOR] Nortriptyline may be unsafe to use during pregnancy. Although there are limited studies in humans, animal studies have shown harmful effects on the developing baby. Your doctor will weigh the benefits and any potential risks before prescribing it to you. Please consult your doctor.
Use in lactation
[CONSULT YOUR DOCTOR] Nortriptyline is probably unsafe to use during breastfeeding. Limited human data suggests that the drug may pass into the breastmilk and harm the baby.
Renal / hepatic impairment
[CAUTION] Nortriptyline should be used with caution in patients with kidney disease. Dose adjustment of Nortriptyline may be needed. Please consult your doctor.
