ওভুলেশন ইনডাকশন, মহিলা বন্ধ্যাত্ব, পলিসিস্টিক ডিম্বাশয় সিন্ড্রোম, অ্যামেনোরিয়া-গ্যালাক্টোরিয়া সিন্ড্রোম, সাইকোজেনিক অ্যামেনোরিয়া খাবারের সাথে বা খাবার ছাড়া নেওয়া যেতে পারে। মৌখিক অ্যানোভুলেটরি বন্ধ্যাত্ব প্রাপ্তবয়স্ক: মাসিক চক্রের 5 তম দিন থেকে শুরু করে বা অ্যামেনোরিয়া থাকলে 5 দিনের জন্য প্রতিদিন 50 মিলিগ্রাম। যদি ডিম্বস্ফোটন না ঘটে, তাহলে 5 দিনের জন্য 100 মিলিগ্রামের দ্বিতীয় কোর্সটি পূর্ববর্তী থেরাপির 30 দিন পরে শুরু হতে পারে। মোট 6টি চিকিত্সা চক্রের পরেও গর্ভধারণ না হলে আরও চিকিত্সার পরামর্শ দেওয়া যেতে পারে। ক্লোমিফেন সাইট্রেট গর্ভাবস্থায়, যকৃতের রোগ, ডিম্বাশয়ের সিস্ট, অনির্ধারিত মূলের রক্তপাত, এন্ডোমেট্রিয়ামের নিওপ্লাজম, অতি সংবেদনশীলতার রোগীদের ক্ষেত্রে নিষেধাজ্ঞাযুক্ত; অস্বাভাবিক রক্তপাত; স্তন্যপান অনিয়ন্ত্রিত থাইরয়েড বা অ্যাড্রিনাল কর্মহীনতা, পিটুইটারি টিউমারের মতো জৈব ইন্ট্রাক্রানিয়াল ক্ষত সহ রোগী। ক্লোমিফেন হাইপোথ্যালামাস এবং পিটুইটারিতে ইস্ট্রোজেনের নেতিবাচক প্রতিক্রিয়া প্রক্রিয়াকে বাধা দেয় যা পিটুইটারি গোনাডোট্রফিক হরমোনের নিঃসরণকে উদ্দীপিত করে যার ফলে ডিম্বস্ফোটন উদ্দীপিত হয়। Clomiphene citrate নিম্নলিখিত অবস্থায় সতর্কতার সাথে ব্যবহার করা উচিত: দীর্ঘ রক্ষণাবেক্ষণ থেরাপি, অ্যালকোহলযুক্ত পানীয় এবং এন্টিডিপ্রেসেন্টস সহ। পলিসিস্টিক ডিম্বাশয়, প্রতিটি চক্র চিকিত্সার আগে ডিম্বাশয়ের সিস্টের উপস্থিতি মূল্যায়ন করুন। জরায়ু ফাইব্রয়েড; চাক্ষুষ ব্যাঘাত ঘটতে পারে যা গাড়ি চালানো বা যন্ত্রপাতি চালানোর ক্ষমতাকে ব্যাহত করবে। একাধিক গর্ভধারণের একটি ন্যূনতম সম্ভাবনা রয়েছে, যা সম্পর্কে রোগীকে সতর্ক করা উচিত। স্তন্যদান: মানুষের দুধে ক্লোমিফেন নির্গত হয় কিনা তা জানা নেই। নার্সিং মহিলাদের সতর্কতার সাথে ব্যবহার করুন। কিছু রোগীর ক্ষেত্রে ক্লোমিফেন স্তন্যপান কমাতে পারে। >10% ডিম্বাশয়ের বৃদ্ধি (14%), ভাসোমোটর ফ্লাশিং (10%) 1-10% পেটে অস্বস্তি (6%), ঝাপসা দৃষ্টি (1.5%), স্তনে অস্বস্তি (2%), বমি বমি ভাব/বমি হওয়া (2%) গর্ভবতী মহিলাদের মধ্যে গর্ভাবস্থার ব্যবহার contraindicated হয়, কারণ এই জনসংখ্যায় চিকিত্সা সুবিধা দেয় না; গর্ভাবস্থার প্রথম দিকে অসাবধানতাবশত প্রশাসন এড়াতে, ডিম্বস্ফোটন এবং/অথবা গর্ভাবস্থা ঘটে কিনা তা নির্ধারণ করতে প্রতিটি চিকিত্সা চক্রের সময় উপযুক্ত পরীক্ষাগুলি ব্যবহার করা উচিত; প্রতিটি চিকিত্সা চক্রের মধ্যে ডিম্বাশয়ের বৃদ্ধি বা ডিম্বাশয়ের সিস্ট গঠন বাদ দেওয়ার জন্য রোগীদের যত্ন সহকারে মূল্যায়ন করা উচিত; থেরাপির পরবর্তী কোর্সটি বিলম্বিত হওয়া উচিত যতক্ষণ না এই অবস্থাগুলি বাদ দেওয়া হয় মহামারীবিদ্যার গবেষণা থেকে পাওয়া মানব ডেটা ক্লোমিফেন সাইট্রেট পেরিকনসেপচুয়াল এক্সপোজার এবং সামগ্রিক জন্মগত ত্রুটির বর্ধিত ঝুঁকি, বা কোনও নির্দিষ্ট অসঙ্গতি স্তন্যদানের মধ্যে আপাত কারণ এবং প্রভাবের সম্পর্ক দেখায় না। ওষুধ মানুষের দুধে নির্গত হয় কিনা তা জানা; যেহেতু অনেক ওষুধ মানুষের দুধে নির্গত হয়, সেহেতু যদি একজন স্তন্যদানকারী মহিলাকে ওষুধ দেওয়া হয় তবে সতর্কতা অবলম্বন করা উচিত; কিছু রোগীর ক্ষেত্রে, থেরাপি স্তন্যপান কমাতে পারে ড্যানাজলের সাথে প্রতিক্রিয়া হ্রাস।

Clomifene inhibits the negative feedback mechanisms of oestrogens in the hypothalamus and pituitary which stimulates the secretion of pituitary gonadotrophic hormones resulting in stimulation of ovulation.

Adult Dose: Oral Anovulatory infertility Adult: 50 mg daily for 5 days starting on the 5th day of menstrual cycle or at any time if there is amenorrhoea. If ovulation does not occur, a 2nd course of 100 mg for 5 days may be used commencing as early as 30 days after the previous therapy. Further treatment may not be recommended if pregnancy has not occurred after a total of 6 treatment cycles.

May be taken with or without food.

Clomiphene citrate should be used with caution in the following conditions: longer maintenance therapy, with alcoholic drinks & antidepressants. Polycystic ovaries, evaluate presence of ovarian cyst before each cycle treatment. Uterine fibroids; visual disturbances may develop which will impair ability to drive or operate machinery. There is a minimal chance of multiple pregnancies, about which the patient should be warned. Lactation: It is not known whether clomiphene is excreted in human milk. Use with caution in nursing women. In some patients, clomiphene may reduce lactation.

Clomiphene citrate is contraindicated during pregnancy, in patients with liver diseases, ovarian cysts, bleeding of undetermined origin, patients with neoplasm of endometrium, hypersensitivity; abnormal bleeding; lactation; uncontrolled thyroid or adrenal dysfunction, patient with an organic intracranial lesions such as pituitary tumor.

Decreased response with danazol.

Pregnancy Use in pregnant women is contraindicated, as treatment does not offer benefit in this population; to avoid inadvertent administration during early pregnancy, appropriate tests should be utilized during each treatment cycle to determine whether ovulation and/or pregnancy occurs; patients should be evaluated carefully to exclude ovarian enlargement or ovarian cyst formation between each treatment cycle; the next course of therapy should be delayed until these conditions have been excluded Available human data from epidemiologic studies do not show apparent cause and effect relationship between clomiphene citrate periconceptual exposure and an increased risk of overall birth defects, or any specific anomaly Lactation It is not known whether drug is excreted in human milk; because many drugs are excreted in human milk, caution should be exercised if drug is administered to a nursing woman; in some patients, therapy may reduce lactation
