Description

Niacin (nicotinic acid) is a medicine used in the treatment of conditions caused due to low levels of vitamin B3 in the body. It helps in maintenance of normal cholesterol level. It also helps in the treatment of symptoms like loss of appetite, weakness, diarrhea, skin inflammation, and mental changes. Niacin (nicotinic acid) can be taken with or without food. It should be taken regularly and at the same time each day to get the most benefit from it. This medicine may be only part of a complete program of treatment that also includes making changes to your diet and taking calcium and vitamin supplements. Learn about the foods you should eat to make sure you get enough vitamins in your diet. This medicine may cause few side effects such as itching, flushing (sense of warmth in the face, ears, neck and trunk), redness of skin, and tingling in some patients. If any of these side effects bothers you, consult with your doctor. Before taking this medicine, tell your doctor if you have any heart, kidney, or liver disease. Some other medicines can decrease the absorption of vitamins, so let your doctor know all other medicines you are taking to make sure you are safe. It is important to ask the advice of you doctor before taking any other vitamin supplements. Niacin (nicotinic acid) stimulates the formation of enzymes for triglyceride metabolism thereby reducing the levels of triglyceride and cholesterol in the body.

Mechanism of action

Nicotinic acid is a derivative of vitamin B3 and is incorporated into coenzymes nicotinamide adenine dinucleotide (NAD) and nicotinamide adenine dinucleotide phosphate (NADP), which are involved in multiple cellular metabolic pathways. Nicotinic acid also reduces total serum cholesterol, LDL, VLDL, and triglycerides, and increases HDL cholesterol. It appears to decrease hepatic synthesis of VLDL, however, the exact mechanisms of nicotinic acid's antilipemic effects are unknown and are unrelated to its role as a vitamin.

Dosage

Adult Dose: Oral Treatment and prophylaxis of nicotinic acid deficiency Adult: For treatment of pellagra: 300-500 mg daily in divided doses. For management of Hartnup disease: 50-200 mg daily. Hyperlipidaemias Adult: Initially, 250 mg once daily in the evening. Increase dose every 4-7 days until desired LDL cholesterol and/or triglyceride level is achieved or dose of 1.5-2 g/day is reached. If hyperlipidaemia is not adequately controlled after 2 mth w/ this dose, it can be increased at 2- to 4-wk intervals to 1 g tid. Max: 6 g daily. As extended-release tab: Initially, 500 mg/day PO at bedtime initially; dose adjusted every 4 weeks on basis of effect and tolerance to therapeutic dose of 1-2 g once daily; not to exceed 1-2 g/day Vasodilation Adult: 100-150 mg 3-5 times daily. As extended-release tab: 300-400 mg 12 hrly. Child Dose: Oral Treatment and prophylaxis of nicotinic acid deficiency Child: For treatment of pellagra: 100-300 mg daily in divided doses.

Administration

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Niacin (nicotinic acid) may be taken with or without food, but it is better to take it at a fixed time.

Side effects

Common: • Itching • Flushing (sense of warmth in the face, ears, neck and trunk) • Erythema (skin redness) • Tingling

Precautions & warnings

[SAFE] Niacin (nicotinic acid) does not usually affect your ability to drive. It is best known to increase "good" cholesterol (HDL). This helps prevent heart attack and stroke. It is better to take with food to avoid an upset stomach. Avoid alcohol, spicy foods, hot beverages or hot baths right before you take Niacin (nicotinic acid). You may feel hot and your skin colour may change after taking Niacin (nicotinic acid). Your doctor may tell you to take an aspirin 30 minutes before you take Niacin (nicotinic acid) to reduce this effect. Notify your doctor if you have unexplained muscle pain.

Contraindications

Niacin is contraindicated in patients with a known hypersensitivity to Niacin or any component of this medication, significant or unexplained hepatic dysfunction, active peptic ulcer disease or arterial bleeding.

Drug interactions

Increased risk of myopathy and rhabdomyolysis w/ concomitant HMG-CoA reductase inhibitors. Reduced bioavailability w/ colestyramine or colestipol. Decreased metabolic clearance w/ aspirin. May potentiate effects of ganglionic blocking agents and vasoactive drugs resulting in postural hypotension.

Food interactions

Use in pregnancy

[CONSULT YOUR DOCTOR] Niacin (nicotinic acid) may be unsafe to use during pregnancy. Although there are limited studies in humans, animal studies have shown harmful effects on the developing baby. Your doctor will weigh the benefits and any potential risks before prescribing it to you. Please consult your doctor.

Use in lactation

[SAFE IF PRESCRIBED] Niacin (nicotinic acid) is safe to use during breastfeeding. Human studies suggest that the drug does not pass into the breastmilk in a significant amount and is not harmful to the baby.

Renal / hepatic impairment

[CONSULT YOUR DOCTOR] There is limited information available on the use of Niacin (nicotinic acid) in patients with kidney disease. Please consult your doctor. However, a cautious use of Niacin (nicotinic acid) is advised in patients with kidney disease as the information available is very limited. [CAUTION] Niacin (nicotinic acid) should be used with caution in patients with liver disease. Dose adjustment of Niacin (nicotinic acid) may be needed. Please consult your doctor. Use of Niacin (nicotinic acid) is not recommended in patients with active liver disease.
