Rivastigmine is used for symptomatic treatment of mild to moderate dementia in Alzheimer's disease and dementia associated with Parkinson's disease. It is a cholinesterase inhibitor that increases acetylcholine levels in the brain, improving memory, awareness, and daily functioning.
Indications
- Mild to moderate dementia of Alzheimer's type
- Dementia associated with idiopathic Parkinson's disease
Dosage
Capsule (Adult):
- Initial: 1.5 mg twice daily (with morning and evening meals)
- Titration: Increase to 3 mg twice daily after 2 weeks; then to 4.5 mg and 6 mg twice daily based on tolerability
- Maintenance: 3-6 mg twice daily (maximum 6 mg twice daily)
Transdermal Patch:
- Initial: 4.6 mg/24 hours
- Titration: Increase to 9.5 mg/24 hours after 4 weeks; then to 13.3 mg/24 hours after additional 4 weeks if needed
Side effects
Common Side Effects:
- Nausea and vomiting (38% and 23%) – especially during dose titration
- Loss of appetite and weight loss
- Dizziness and headache
- Excessive sweating and weakness
- Abdominal pain and diarrhea
Precautions & warnings
- Caution in sick sinus syndrome or conduction defects
- Caution in asthma or obstructive pulmonary disease
- Caution in peptic ulcer and urinary obstruction
- Monitor body weight regularly
- Use in pregnancy only if clearly necessary
