Indications
Used to lower high cholesterol, mixed dyslipidaemia and high triglycerides, and to prevent cardiovascular disease and stroke.

Pharmacological class
HMG-CoA reductase inhibitor
Mechanism of action
Inhibits HMG-CoA reductase, the key enzyme of hepatic cholesterol synthesis, upregulating LDL receptors and lowering 'bad' LDL cholesterol.
Dosage
Usually 5–20 mg once daily; maximum 40 mg (specialist use only). Typically start at 5–10 mg. Asian patients should start at a lower dose.
Administration
Once daily, any time of day, with or without food.
Missed dose
Take when remembered; skip if the next dose is near.
Side effects
Muscle pain, headache, nausea, raised liver enzymes; rarely rhabdomyolysis.
Precautions & warnings
Monitor liver tests and muscle symptoms; caution in hypothyroidism and heavy alcohol use.
Contraindications
Active liver disease, pregnancy, breastfeeding, myopathy.
Drug interactions
Ciclosporin, gemfibrozil, some antivirals, warfarin (INR), antacids.
Food interactions
No significant food interaction; avoid excess alcohol.
Use in pregnancy
Contraindicated in pregnancy (Category X).
