Description

Carbidopa + Levodopa is a combination of two medicines used to treat Parkinson’s disease. It is one of the most effective medications to relives symptoms of Parkinson’s disease such as tremor, muscle stiffness and difficulty moving. Carbidopa + Levodopa can be taken without food but it is better to avoid high protein diet and dairy products while taking this medicine. However, it is advised to take it at the same time each day as this helps to maintain consistent level of medicine in the body. Take this medicine in the dose and duration advised by your doctor and if you have missed a dose, take it as soon as you remember it. You should never skip any doses and try to finish the full course of treatment even if you feel better. Do not stop taking medicine until told by your doctor as it can cause harmful effects. This medicine may cause side effects like nausea, vomiting, dryness in mouth, constipation, and headache. Initially, this medicine may cause a sudden drop in blood pressure when you change positions, so it's better to rise slowly if you have been sitting or lying down. It causes a sudden onset of dizziness and sleepiness, so do not drive or do anything that requires mental focus until you know how this medicine affects you. Take caution while taking medicine if you are suffering from liver or kidney disease. Your doctor should also know about all other medicines you are taking as many of these may make this medicine less effective or change the way it works. Inform your doctor if you are pregnant, planning pregnancy or breastfeeding. Carbidopa + Levodopa is a combination of two medicines: Levodopa and Carbidopa which treats Parkinsonism. Levodopa is a dopamine precursor which gets converted to dopamine, a chemical messenger that controls movement in the brain. Carbidopa is a peripheral decarboxylase inhibitor which prevents breakdown of levodopa allowing it to enter the brain and increase dopamine levels.

Mechanism of action

Levodopa is the metabolic precurosor of dopamine, it crosses the blood-brain barrier and is converted to dopamine in the brain. Carbidopa increases the amount of levodopa that is transported into the CNS by inhibiting the decarboxylation of peripheral levodopa.

Dosage

Adult Dose: Parkinson Disease & Parkinson like Disorders Adult: PO: Immediate release: 25 mg/100 mg q8hr or 10 mg/100 mg PO q6-8hr initially; levodopa may be increased by 100 mg/day every 1-2 days Carbidopa should be 70-100 mg/day but not to exceed 200 mg/day; levodopa not to exceed 800 mg/day CR tablet: 50 mg/200 mg PO q12hr initially; may be increased up to 1600 mg/day of levodopa; doses must be given at least 6 hours apart Hepatic impairment: Safety and efficacy not established; use with caution Child Dose: Safety and efficacy not established Renal Dose: Renal impairment: Safety and efficacy not established; use with caution

Administration

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Carbidopa + Levodopa is to be taken empty stomach. Avoid Carbidopa + Levodopa with high protein meals such as cottage cheese, Swiss cheese, protein powder, eggs and milk.

Precautions & warnings

[UNSAFE] Carbidopa + Levodopa may cause side effects which could affect your ability to drive. As Carbidopa + Levodopa may cause dizziness, drowsiness, double vision, which may affect your ability to react which may affect your ability to drive. You have been prescribed Carbidopa + Levodopa to treat your symptoms of Parkinson's disease. It can be taken with or without food or as directed by your doctor. Reduce/avoid a high-protein diet (e.g. meat, poultry, and dairy products) while you are taking the medicine as it can reduce the amount of medication absorbed by your body. Be cautious while driving or doing anything that requires concentration as Carbidopa + Levodopa can make you very sleepy. When you start the medication, get out of bed slowly, as the medication may cause dizziness, lightheadedness or fainting. Do not stop taking the medication without talking to your doctor. It may result in a serious syndrome that causes fever, stiff muscles, unusual body movements, sweating, and confusion. Talk to your doctor in case you develop hallucinations or other changes in your behavior.

Contraindications

Lactation, narrow angle glaucoma, melanoma, psychosis, severely decompensated endocrine.

Drug interactions

Pyridoxine reverses effects of levodopa. Type-B MAOIs have synergistic effect. Effect reduced by phenothiazines, haloperidol, reserpine, pyridoxine, diazepam, oxazepam, chlordiazepoxide, phenobarbitone. Effects of levodopa enhanced by carbidopa, amantadine, anticholinergics, amphetamine. Effects of sympathomimetic agents enhanced. Potentially Fatal: Enhances hypotensive effects of bethanidine, bretylium, guanethidine. Hypertensive crises with furazolidone or MAO inhibitors.

Food interactions

[UNSAFE] It is unsafe to consume alcohol with Carbidopa + Levodopa.

Use in pregnancy

Use in lactation

[SAFE IF PRESCRIBED] Carbidopa + Levodopa is probably safe to use during breastfeeding. Limited human data suggests that the drug does not represent any significant risk to the baby. Partial to complete suppression of lactation was seen in mothers on the treatment with Carbidopa + Levodopa, but when the treatment was stopped, lactation returned to normal levels.

Renal / hepatic impairment

[SAFE IF PRESCRIBED] Carbidopa + Levodopa is probably safe to use in patients with kidney disease. Limited data available suggests that dose adjustment of Carbidopa + Levodopa may not be needed in these patients. Please consult your doctor.
