Sacubitril + Valsartan is an ARNI (angiotensin receptor neprilysin inhibitor) used to reduce the risk of cardiovascular death and hospitalization in patients with chronic heart failure (NYHA class II-IV). It relaxes blood vessels and makes it easier for the heart to pump blood.
Indications
- Chronic heart failure (NYHA class II-IV) – reduce cardiovascular death and hospitalization
- Pediatric heart failure (≥1 year)
Dosage
Adult Heart Failure:
- Starting: 49/51 mg twice daily
- Target maintenance: Double to 97/103 mg twice daily after 2-4 weeks (as tolerated)
- Reduced starting (not on ACEi/ARB, low dose, severe renal, moderate hepatic): 24/26 mg twice daily
Pediatric (≥1 year):
- <40 kg: 1.6→2.3→3.1 mg/kg
- 40-<50 kg: 24/26→49/51→49/51 mg
- ≥50 kg: 49/51→72/78→97/103 mg
Side effects
Common Side Effects:
- Angioedema
- Hypotension
- Impaired renal function
- Hyperkalemia
- Cough

- Orthostatic hypotension
Precautions & warnings
- Do not use in patients with history of angioedema
- Do not use with ACE inhibitors (36-hour washout)
- Do not use with aliskiren in diabetic patients
- Not recommended in severe hepatic impairment (Child-Pugh C)
- Unsafe in pregnancy; avoid in lactation
