Pharmacology
Aclidinium bromide is a long-acting anticholinergic that inhibits M3 receptors causing smooth muscle relaxation. Formoterol fumarate is a selective long-acting beta2-agonist that increases cAMP levels leading to bronchial relaxation.
Mechanism of action
Provides complementary bronchodilation by blocking cholinergic bronchoconstriction (Aclidinium) and stimulating beta2-adrenergic receptors (Formoterol) in the airways.
Dosage
400 mcg/12 mcg twice daily (one inhalation capsule in the morning and one in the evening) via an inhalation device.
Administration
For oral inhalation only using the appropriate inhaler device. Do not swallow capsules.
Missed dose
Take as soon as remembered. If it is almost time for the next dose, skip the missed dose. Do not take extra doses.
Side effects
Common adverse effects include upper respiratory tract infection and headache. Tremor, palpitations, dry mouth, or urinary retention may occur.
Precautions & warnings
Not for acute bronchospasm or acute COPD exacerbations. Use with caution in convulsive disorders, thyrotoxicosis, diabetes, narrow-angle glaucoma, or prostatic hyperplasia.
Contraindications
Contraindicated in asthma without concomitant inhaled corticosteroids (ICS) and in patients with hypersensitivity to any component.
Drug interactions
Concomitant use with xanthines, steroids, or non-potassium-sparing diuretics may worsen hypokalemia. Extreme caution with MAOIs, TCAs, and beta-blockers.
Food interactions
No known interactions with food as it is administered via inhalation.
Use in pregnancy
No adequate and well-controlled studies in pregnant women. Use only if potential benefit justifies potential risk.

Use in lactation

Overdose effects
