Description

Alendronic Acid + Vitamin D3 is prescribed to treat osteoporosis. It prevents bone loss and helps in thickening of the bones. It also strengthens the muscles to support the body. It is a potent medicine used to prevent bone loss in women after menopause by rebuilding their bones. Alendronic Acid + Vitamin D3 is best taken in an empty stomach. You should take it regularly at a fixed time each day. Taking it at the same time every day will help you remember to take it. It may take several days to get the full benefit of this drug. Do not stop taking the medicine until your doctor tells you it is alright to stop. This medicine may only be part of a treatment program that may also include changes to your diet and taking other medicines and vitamin supplements. The most common side effects of this medicine include headache, back pain, musculoskeletal (bone, muscle or joint) pain, indigestion, heartburn, and diarrhea. These are generally mild and usually resolve on their own. If they do not go away or get worse, you should let your doctor know. Pharmacology

Alendronic acid is a bisphosphonate that acts as a specific inhibitor of osteoclast-mediated bone resorption . It preferentially localizes to sites of bone resorption and inhibits osteoclast activity, resulting in bone formation exceeding bone resorption and progressive gains in bone mass . Vitamin D3 (colecalciferol) is the natural precursor of the calcium-regulating hormone calcitriol, which enhances intestinal absorption of calcium and phosphate and supports bone mineralization . Vitamin D insufficiency leads to negative calcium balance, increased parathyroid hormone levels, bone loss, and increased fracture risk .

Mechanism of action

Alendronic Acid + Vitamin D3 is a combination of two medicines: Alendronic Acid and Vitamin D3 which treat osteoporosis. Alendronic Acid is a bisphosphonate. It works by suppressing the activity of osteoclasts, cells that cause destruction of bone. This strengthens the bone and minimizes the risk of fractures. Dosage

Adult Dose: Upper GI disorders (discontinue if symptoms worsen); history of ulcers, active GI bleeding. Correct vitamin D and calcium deficiency before starting therapy. To be taken half an hr before breakfast and remain upright for at least 30 minutes after admin. Not recommended for use in patients with CrCl <35 ml/min. Renal Dose: Alendronic acid reduces bone resorption by inhibiting the action of osteoclasts.

Administration

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Alendronic Acid + Vitamin D3 is to be taken empty stomach.

Missed dose

• If a weekly dose is missed: Take 1 tablet the morning after you remember, then return to your regular weekly schedule. • Do not take 2 tablets on the same day.

Alendronic Acid + Vitamin D3 is used in the treatment of osteoporosis Take it on the same day each week, in the morning, at least 30 minutes before eating or drinking anything. Maintain an upright posture for 30 minutes after taking the medication so the medicine doesnot irritate your food pipe. Take care of your teeth and jaw while taking the Alendronic Acid + Vitamin D3 and for regular dental checkup while taking Alendronic Acid + Vitamin D3. Take calcium and vitamin enriched diet. Side effects such as headache, back pain and indigestion may occur.

Contraindications

• Esophageal abnormalities that delay esophageal emptying, such as stricture or achalasia. • Inability to sit upright or stand for at least 30 minutes. • Hypersensitivity to alendronic acid, cholecalciferol, or any of its components. • Hypocalcemia (low blood calcium levels).

Drug interactions

Hypocalcaemia; oesophageal abnormalities and factors which delay oesophageal emptying; severe renal impairment; hypersensitivity; inability to stand or sit upright for <30 min. Pregnancy, lactation.

Food interactions

Caution is advised when consuming alcohol with Alendronic Acid + Vitamin D3. Please consult your doctor.

Use in pregnancy

Overdose effects

Alendronate overdose may cause hypocalcemia, hypophosphatemia, and upper gastrointestinal problems such as stomach upset, heartburn, and esophagitis. Milk or antacids may help bind alendronate. Do not induce vomiting, and keep the patient upright. Symptoms of vitamin D toxicity may include hypercalcemia, hypercalciuria, anorexia, nausea, polyuria, and weakness.
