ব্যাকটেরিয়াল কানের সংক্রমণ প্রাপ্তবয়স্কদের বহিরাগত ওটিক/অরাল ওটিটিস: কানের মধ্যে 2-3 ফোঁটা প্রবেশ করান। হেপাটিক বৈকল্য: ডোজ কমানোর প্রয়োজন হতে পারে। রেনাল বৈকল্য: ডোজ কমানোর প্রয়োজন হতে পারে। ওষুধের প্রতি সংবেদনশীলতার ইতিহাস; ছিদ্রযুক্ত কানের ড্রাম। হার্পিস সিমপ্লেক্স এবং কানের অন্যান্য ভাইরুল অবস্থা, মাইকোসিস। ক্লোরামফেনিকল ব্যাকটেরিয়াল রাইবোসোমের 50s সাবইউনিটে আবদ্ধ হয়ে ব্যাকটেরিয়া প্রোটিন সংশ্লেষণকে বাধা দেয়, এইভাবে পেপটাইডিল ট্রান্সফারেজ দ্বারা পেপটাইড বন্ড গঠন প্রতিরোধ করে। এইচ. ইনফ্লুয়েঞ্জা, এন. মেনিনজিটিডিস এবং এস. নিউমোনিয়ার বিরুদ্ধে এটির ব্যাকটিরিওস্ট্যাটিক এবং ব্যাকটেরিয়াঘটিত উভয়ই প্রভাব রয়েছে। দীর্ঘায়িত ব্যবহার এড়িয়ে চলুন। স্থানীয় অতিসংবেদনশীলতা প্রতিক্রিয়া দেখা দিলে অবিলম্বে বন্ধ করুন। কানের ড্রপ: অটোটক্সিসিটি। রক্তাল্পতা রোগীদের মধ্যে আয়রন এবং ভিটামিন বি 12 এর প্রভাব কমে যায়। ফেনোবারবিটোন এবং রিফাম্পিন ক্লোরামফেনিকলের কার্যকারিতা হ্রাস করে। মৌখিক গর্ভনিরোধকগুলির ক্রিয়াকে বাধা দেয়। সম্ভাব্য মারাত্মক: ওরাল অ্যান্টিকোয়াগুলেন্ট, ওরাল হাইপোগ্লাইসেমিক এজেন্ট, ফেনাইটোইনের প্রভাব বাড়ায়। অস্থি মজ্জার কার্যকারিতা হ্রাস করে এমন ওষুধের সাথে একযোগে প্রশাসন এড়িয়ে চলুন।

Bacterial Ear infections
Mechanism of action
Chloramphenicol inhibits bacterial protein synthesis by binding to 50s subunit of the bacterial ribosome, thus preventing peptide bond formation by peptidyl transferase. It has both bacteriostatic and bactericidal action against H. influenzae, N. meningitidis and S. pneumoniae.
Dosage
Adult Dose: Otic/Aural Otitis externa Adult: Instill 2-3 drops into the ear bid-tid. Hepatic impairment: Dose reduction may be required. Renal Dose: Renal impairment: Dose reduction may be required.
Side effects
Ear drops: Ototoxicity.
Precautions & warnings
Avoid prolonged use. Discontinue immediately if local hypersensitivity reactions occur.
Contraindications
History of hypersensitivity to the drug; Perforated ear drum. Herpes simplex and other virul conditions of the ear, mycosis.
Drug interactions
Decreased effects of iron and vitamin B12 in anaemic patients. Phenobarbitone and rifampin reduce efficacy of chloramphenicol. Impairs the action of oral contraceptives. Potentially Fatal: Increases the effect of oral anticoagulants, oral hypoglycaemic agents, phenytoin. Avoid concomitant administration with drugs that depress bone marrow function.
Renal / hepatic impairment
Renal impairment: Dose reduction may be required.
